Subject: MALFUNCTION OF HDR CONTROL CONSOLE DURING PATIENT TREATMENT
Discussion:
Region I received notification on March 12, 1999, that a High Dose Rate
Afterloader (HDR) treatment console froze during a patient treatment. On
March 11, 1999 at Beth Israel Hospital in Passaic, NJ, the operator noted
the treatment time had passed by 50 seconds and the console did not
indicate that the treatment had ended. The operator pushed the emergency
off button and confirmed that the radiation monitor indicated the source
had returned to the shield. The licensee called Nucletron Customer
Service, the HDR vendor and their engineers identified that the treatment
had been completed as intended by using a hand held terminal attached to
the HDR unit to read the treatment executed parameters. Nucletron
replaced the motherboard and power supply and tested the system.
Regional Action:
This information is preliminary and Region I is discussing with the
licensee the cause of the machine failure and actions to prevent
recurrence. Region I will perform an inspection on March 16, 1999, to
follow up on the licensee's actions relative to this event.
