Headquarters Daily Report MARCH 16, 1999 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS X REGION I X REGION II X REGION III X REGION IV X PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION I MARCH 16, 1999 Licensee/Facility: Notification: MR Number: 1-99-0011 Beth Israel Hospital Date: 03/12/99 Passaic,New Jersey Dockets: 03002465 License No: 29-03047-01 Subject: MALFUNCTION OF HDR CONTROL CONSOLE DURING PATIENT TREATMENT Discussion: Region I received notification on March 12, 1999, that a High Dose Rate Afterloader (HDR) treatment console froze during a patient treatment. On March 11, 1999 at Beth Israel Hospital in Passaic, NJ, the operator noted the treatment time had passed by 50 seconds and the console did not indicate that the treatment had ended. The operator pushed the emergency off button and confirmed that the radiation monitor indicated the source had returned to the shield. The licensee called Nucletron Customer Service, the HDR vendor and their engineers identified that the treatment had been completed as intended by using a hand held terminal attached to the HDR unit to read the treatment executed parameters. Nucletron replaced the motherboard and power supply and tested the system. Regional Action: This information is preliminary and Region I is discussing with the licensee the cause of the machine failure and actions to prevent recurrence. Region I will perform an inspection on March 16, 1999, to follow up on the licensee's actions relative to this event. Contact: T. MR Number: 4-99-0011 Diablo Canyon 1 Date: 03/15/99 Avila Beach,California Resident Inspector Dockets: 50-275 PWR/W-4-LP Subject: RESTART FROM REFUELING OUTAGE 9 Discussion: At 10:18 p.m. (PST) on March 13, 1999, the Diablo Canyon Unit 1 reactor reached criticality. At 12:35 a.m. on March 15, 1999, Unit 1 paralleled to the grid. This completed the ninth refueling outage, which lasted 35 days, 9 hours, and 52 minutes. Major activities accomplished during the outage included: refueling; inspection and maintenance of the main turbine and generator; inspection of the steam generators; replacement of three of the five containment fan cooling units and overhaul of the other two; 10-year inservice inspection of the reactor vessel nozzles; replacement of Main Feedwater Pump 1-1; and replacement of all of the reactor control rod guide tube split pins.
