NEW ISSUE. The vendor, Automatic Switch Company, (ASCO), reports a
potential manufacturing deficiency in certain pump and amplifier
assemblies using ASCO General Controls Hydramotor actuators. The vendor
was notified by the Nine Mile Point Unit 2 licensee that such an actuator
failed during post-installation tests. This condition may result in
(1) failure of the actuator piston to react when the actuator is
energized or (2) return of the piston to the spring-extended position
when the actuator is energized. The vendor claims insufficient knowledge
of the actual installation and operating conditions of these actuators to
determine existence of substantial safety hazard, as defined in 10 CFR
Part 21, but the vendor continues to identify purchasers of affected
pumps, amplifiers, and complete actuators in which these assemblies may
be used. The vendor intends to notify these purchasers.

NEW ISSUE. On February 9, 1999, the Beaver Valley Unit 1 licensee made
an event notification identifying nonconservative inputs in the
radiological analysis of an accident-initiated increase in iodine
concentration (spike) in the reactor coolant system. On February 19,
1999, the licensee determined that this matter was reportable under 10
CFR Part 21. The licensee continues to evaluate this issue.
