Subject: PLANNED UNIT 2 OUTAGE REPLACE CABLE AND REPAIR MANWAY COVERS
Discussion:
On 2/6/99, the inspectors observed a planned shutdown of Unit 2 to
replace a faulty cable to the 2N44 nuclear instrument and to repair steam
leaks on the 22 steam generator manways. Subsequent to placing the unit
in Hot Shutdown, an improperly implemented Temporary Change Notice (TCN)
to the Unit 2 Shutdown Procedure (2C1.3) led to the reactor operator
withdrawing the shutdown banks prior to performing the T.S. required
surveillance procedure 2035D, "Reactor Protection Logic Testing." This
surveillance is required to be performed following a shutdown, prior to
making the control rod drive system capable of rod withdrawal. After the
licensee discovered that this had occurred, the shutdown banks were
inserted, the reactor trip breakers were opened, and the SP 2035D was
performed prior to reclosing the reactor trip breakers.
On 2/7/99, the NI cable and steam generator manway repairs were completed
successfully. The inspectors observed the Unit 2 reactor startup and no
other discrepancies were noted.
