AGREEMENT STATE REPORT - LOST PACKAGE

The following information was provided by the Illinois Emergency Management Agency (the Agency) via phone and email:

"The Agency was contacted by GE Healthcare in Arlington Heights, IL, to report that a package containing a diagnostic radiopharmaceutical was lost in transit. The package contains one vial of In-111 diethylenetriamine pentaacetate (DTPA) product calibrated at 1.5 millicuries for June 17, 2026. This package and its contents were initially scanned as received at the common carrier hub in Memphis, TN, but have since shown no movement.

"The quantity of radioactive material has decayed (.0343 millicuries) and does not represent a significant hazard to the public. There is no indication of intentional theft or diversion.

"The radiopharmaceutical was packaged for shipment on June 12, 2026 (activity at shipment 5.210 millicuries), for delivery to the customer by June 15, 2026. Last documented scan shows no movement after an arrival scan at the common carrier Memphis hub on the early morning of June 13, 2026, at 1213 [CDT]. GE Healthcare contacted the customer, confirmed that the package was not received. On June 18, 2026, following communication with the common carrier on the package's status, common carrier personnel confirmed that a search had been initiated.

"This report will be updated when additional information becomes available."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. AGREEMENT STATE REPORT - MEDICAL EVENT

The following information was provided by the Ohio Department of Health via email:

"On June 3, 2026, the licensee notified the Ohio Department of Health (ODH) that its high-dose-rate remote afterloader unit failed to fully retract into the shielded position during a fraction treatment, causing the treatment plan to stop. The source housing measured 4.5 microsieverts per hour. The patient was surveyed, confirmed to be at background, and removed from the treatment room.

"That same day, the licensee had the unit serviced, performed quality spot checks, created a new treatment plan for the remaining channels, and delivered the remainder of the dose using an additional fraction. The licensee initially informed ODH that this constituted a continued treatment and not a medical event.

"On June 18, 2026, a new ODH supervisor assumed responsibility for the incident and requested additional information. The supervisor determined that the fraction ended when the unit failed, since servicing was required and a new treatment plan had to be created. On June 22, 2026, the licensee informed ODH that the patient had received only 36 percent of the prescribed dose, a 64-percent deviation from the intended fraction dose.

"This incident is now considered a reportable medical event. The licensee will notify the patient and provide a written report. The patient was not harmed. A report has been submitted to NMED."
