AGREEMENT STATE REPORT - UNATTENDED RADIOGRAPHY CAMERA

The following information was provided by the Louisiana Department of Environmental Quality (LDEQ) via email:

"On March 3, 2026, the site work coordinator at the ExxonMobil refinery in Baton Rouge reported that a radiography camera had been found unattended. The camera was discovered by an employee of IRIS Inspection Company.

"The licensee crew consisted of two state-carded radiographers and one instructor radiographer. The crew entered the unit, set up the barricade, and completed one exposure. After completing the exposure, they took down the barricade but inadvertently left the camera behind in the unit.

"The camera remained unattended for approximately one hour and was returned to Acuren at approximately 0945 CST."

THIS MATERIAL EVENT CONTAINS A 'Category 2' LEVEL OF RADIOACTIVE MATERIAL

Category 2 sources, if not safely managed or securely protected, could cause permanent injury to a person who handled them, or were otherwise in contact with them, for a short time (minutes to hours). It could possibly be fatal to be close to this amount of unshielded radioactive material for a period of hours to days. These sources are typically used in practices such as industrial gamma radiography, high dose rate brachytherapy and medium dose rate brachytherapy. DEGRADED REACTOR VESSEL CLOSURE HEAD PENETRATION

The following information was provided by the licensee via phone and email:

"At 1717 CDT on March 8, 2026, during the Byron Unit 1 refueling outage, it was determined that the control rod drive mechanism (CRDM) penetration 31 was degraded because liquid penetrant testing performed on the embedded flaw repair weld identified an unacceptable rounded indication, in accordance with American Society of Mechanical Engineers (ASME) Section III acceptance standards and an NRC-approved licensee relief request for a previously performed embedded flaw repair. Required repairs will be completed in accordance with the ASME code of record prior to returning the vessel head to service. Therefore, this event is being reported as an eight-hour, non-emergency notification per 10 CFR 50.72(b)(3)(ii)(A).

"There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified."

MEDICAL EVENT

The following information was provided by the licensee via phone:

On March 3, 2026, at 1038 EST, a 200 mCi Lu-177 Lutathera treatment for a metastatic neuroendocrine tumor had commenced when the patient informed the treatment team that he had received an injection that morning. It was determined that the patient had been administered an octreotide injection. The physician ordered an immediate stop to the Lutathera infusion as the octreotide would prevent take-up of the Lu-177. Only 19 mCi of the planned 200 mCi dose was administered.

A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.

The individual's unescorted access has been immediately placed on administrative hold until the denial process has been completed per station procedures.
