AGREEMENT STATE REPORT - STUCK OPEN GAUGE SHUTTERS
The following is a summary of information provided by the Utah Division of Waste Management and Radiation Control (DWMRC) via email:
On February 24, 2026, at approximately 1500 MST, the licensee reported that two fixed gauge shutters were found stuck open during an annual shutter check. The licensee has scheduled a vendor to replace the gauges.
The licensee did not provide DWMRC with any information about the device types or radioisotopes involved at the time of the report. DWMRC is awaiting receipt of this information from the licensee.

AGREEMENT STATE REPORT - LOST IODINE SEED
The following information was received from the New York State Department of Health, Bureau of Environmental Radiation Protection (the Department) via email:
"On January 29, 2026, the Department was notified of a missing iodine-125 localization seed (48 microcuries) at a medical facility located in New York State.
"The I-125 localization seed was removed from storage for implantation into a patient on October 30, 2025. During the quarterly inventory performed on December 30, 2025, the licensee discovered that the I-125 localization seed was not in the storage facility. Surveys of use areas were performed but the seed was not recovered. The facility believes that the seed (which was pre-loaded into a needle) was discarded in the sharps container and was still within the lead shielding."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL
Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. AGREEMENT STATE REPORT - FOUND SOURCE
The following information was received from the California Department of Public Health, Radiologic Health Branch (RHB) via email:
"On February 24, 2026, a truck trailer that had triggered the radiation alarm at the WM-Azusa Special Waste Services (Azusa Landfill) on February 19, was dumped to allow Los Angeles County Public Health Radiation Management personnel to search for a suspected Ra-226 source in a load of waste glass fragment material based on RHB Identifier R400 identification of Ra-226. The search did not find an intact Ra-226 source as expected. Instead, the search found fragments of Ra-226 in a portion of the dumped load. Since the recovered Ra-226 material appears to have significantly exceeded 1000 times the Part 20, Appendix C value, RHB is reporting this discovery. The RHB will be visiting the glass recycling company in an attempt to determine the origin of the Ra-226 material and any potential significant exposures to personnel of the glass recycling company (Strategic Materials, Inc), the truck driver, and landfill personnel."

MEDICAL EVENT
The following information was provided by the licensee via phone:
A patient was undergoing a Y-90 TheraSphere therapy procedure. The treatment had an issue during the infusion procedure which resulted in an actual dose that was 58 percent of the prescribed dose. The target organ was the liver, and the prescribed activity was approximately 71 mCi. The patient and physician were notified. The cause is still under investigation.
This event is being reported as a medical event per 10 CFR 35.3045(a)(1)(i).
A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
The following information was provided by the licensee via phone:
