EN Revision Text: AGREEMENT STATE REPORT - DOSE TO AN EMBRYO

The following is a summary of information provided by the Iowa Department of Health and Human Services (the Department) via phone and email:

On January 7, 2026, a 150.4 mCi I-131 treatment was administered to a female who had tested negative on a serum pregnancy test two days before, on January 5, 2026. On January 9, 2026, a follow-up serum pregnancy test came back negative. The patient took a pregnancy test (urine) at home that day which came back positive. A second follow-up serum pregnancy test was administered on January 12, 2026, and returned a positive result.

The physician notified the licensee radiation safety officer on January 13, 2026, and the Department was notified on January 14, 2026.

Investigation into the event still ongoing. The hospital is working to calculate/determine what the dose to the embryo was. The Department will perform a reactive inspection/meeting on January 20, 2026.

A reactive inspection was performed on January 20, 2026. Post-treatment scans showed activity only in the patient's thyroid. The calculated dose to the embryo/fetus was 400 mGy. Potential impacts to the embryo/fetus have been evaluated with recommendations for post-treatment care.

EN Revision Text: LOST RADIOACTIVE MATERIAL

The following is a summary of information provided by the licensee via phone:

On January 16, 2026, at approximately 0900 EST, it was discovered that a 202.2 mCi lutetium-177 dose was missing, during preparations to treat a patient. The dose had been delivered to the facility on January 15, 2026. At the time of check in, the dose rate at one meter was 0.05 mr/h. The licensee believes that the radioactive material, shielded in its original shipping vial, was not removed from its shipping container prior to the packaging being discarded with the regular trash. The facility has an exit portal monitor, but no alarm was generated.

The facility's waste management company was notified.

A follow up report will be made in 30 days.

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. AGREEMENT STATE REPORT - VEHICLE INCIDENT

The following is a summary of information provided by the Louisiana Department of Environmental Quality (LDEQ) via email:

On January 17, 2026, at 0030 CST, an Ir-192 brachytherapy source (4 curies) owned by Alpha Omega was involved in a vehicle incident on I-10 westbound near Whiskey Bay (mile marker 131). The source was being returned for disposal by a third-party carrier when a crash involving two tractor trailers resulted in a fire.

The source remained secure in its original shipping package throughout the incident. No radioactive release occurred, and no personnel were overexposed. LDEQ was notified immediately, and emergency response teams managed the situation. The source has been retrieved by Alpha Omega and will proceed to its intended destination. Further details will be provided after the holiday weekend.
