AGREEMENT STATE REPORT - MEDICAL EVENT
The following is a summary of information provided by the New York City Department of Health (NYC DOH) via email:
Only 63 Gy of a planned 151 Gy treatment of Y-90 microspheres to the liver was delivered, or 42 percent of the intended dose. The licensee identified the likely cause as a faulty connection between the tubing in the administration kit and the catheter.
The treatment occurred on November 26, 2025, and was reported to NYC DOH on December 1, 2025.

The following information was provided by the Louisiana Department of Environmental Quality (LDEQ) via email:
"On January 07, 2026, Chalmette Refining, LLC contacted LDEQ concerning a stuck shutter on a fixed gauge identified on January 6, 2026. The shutter is stuck in the open position. The fixed gauge is located on the acid evacuation drum. The licensee has determined that it will be replacing the fixed gauge housing and source with a new fixed gauge housing and source."
