EN Revision Text: AUTOMATIC REACTOR TRIP
The following information was provided by the licensee via phone and email:
"At 1552 EDT on October 14, 2025, with Unit 1 in mode 1 at 100 percent power, the reactor automatically tripped due to a turbine trip. The trip was not complex with all systems responding normally post-trip.
"Operations responded and stabilized the plant. Decay heat is being removed by the emergency feedwater system.
"Due to the reactor protection system actuation while critical, this event is being reported as a 4-hour, non-emergency notification per 10 CFR 50.72(b)(2)(iv)(B) and an 8-hour, non-emergency notification per 10 CFR 50.72(b)(3)(iv)(A) for valid actuation of the reactor protection and emergency feedwater systems. There was no impact on the health and safety of the public or plant personnel. The NRC resident inspector has been notified."
The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:
A generator field breaker lockout occurred. In addition, a small fire was observed in the generator field breaker and extinguished in less than 15 minutes by the onsite fire brigade. The unit is stable in hot standby with emergency feedwater and main condenser steam dumps in-service.

The following information was provided by the Florida Bureau of Radiation Control (BRC) via email:
"Florida BRC received notification of a medical event which occurred at UF Health Jacksonville. A patient received an I-125 seed for breast localization surgery. The seed was implanted on Wednesday October 15, 2025, and was intended to be removed on October 16, 2025.
"On October 16, 2025, the patient required an emergency surgery, which included receiving a stent, resulting in the patient's inability to receive additional surgeries for approximately 3 months. The patient and physician are aware of the situation, and the facility is working towards determining non-surgical options.
"The NRC has received notification due to the expectation that the received dose will be greater than 20 percent of the intended dose. The facility has been requested to provide the patient's expected dose and received dose. The incident, and the NRC, will be updated when this information is received."

FITNESS FOR DUTY (FFD) EVENT
The following information was provided by the licensee via phone and email:
"On 10/25/25, at approximately 1230 EDT, 3 empty alcohol bottles were found in the protected area by a contract employee. Site security was notified and took possession of the empty bottles which were removed from the protected area. The individual who accidentally brought in the empty alcohol bottles with other non-alcoholic empty bottles was tested for FFD and was negative. This event is reportable in accordance with 10 CFR 26.719(b).
"The NRC Resident Inspector has been notified."

"On 10/25/25, at approximately 1230 EDT, 3 empty alcohol bottles were found in the protected area by a contract employee. CONTROL ROOM ENVELOPE INOPERABLE
The following information was provided by the licensee via phone and email:
"On October 26, 2025, at 0040 CDT, the Waterford 3 Steam Electric Unit 3 operating crew declared the control room envelope inoperable in accordance with technical specification (TS) 3.7.6.1 due to the discovery of a hairline crack in the station air supply header in the passageway outside the control room.
"Operations entered TS 3.7.6.1 action b. TS 3.7.6.1 action b was complied with by closing manual valves SA-502 and SA-514, thus isolating the section of piping that developed the crack. The station air leak was isolated at time 0120. There was no impact on the health and safety of the plant or plant personnel.
"This event is reportable pursuant to 10 CFR 50.72(b)(3)(v)(D), event or condition that could have prevented fulfillment of a safety function of structures or systems that are needed to `(D) mitigate the consequences of an accident,' due to the control room envelope being inoperable."
The licensee notified the NRC Resident Inspector.

AGREEMENT STATE REPORT - LOSS OF CONTROL OF RADIOACTIVE MATERIAL
The following is a summary of information provided by the Georgia Radioactive Material Program via phone and email:
On October 13, 2025, Applied Technical Services, LLC (ATS) shipped a radiography camera containing a sealed source to QSA Global via common carrier overnight delivery. The shipment originated from Augusta, Georgia and was destined for Burlington, Massachusetts.
On October 21, 2025, ATS was notified by QSA Global that they had yet to receive the device. ATS followed up with the common carrier to attempt to locate the device and get further status. Using the package tracking number, it was determined that the package arrived at the common carrier's sorting facility in Memphis, Tennessee on October 13, 2025. After further investigation, the package was located at the common carrier facility and will be shipped to QSA Global.

VALID REACTOR PROTECTION SYSTEM ACTUATION WHILE SHUTDOWN
The following information was provided by the licensee via phone and email:
"At 0117 CDT on 10/27/2025, with Unit 2 in mode 3 at 0 percent power, an actuation of the RPS system occurred during restoration of the scram instrument volume high level bypass switch to normal. The cause of the RPS system actuation was a valid high level in the scram instrument volume. All control rods had been previously inserted and the RPS system automatically initiated a scram signal as designed when the scram instrument volume high level signal was received.
"This event is being reported in accordance with 10 CFR 50.72(b)(3)(iv)(A) as an event that results in a valid actuation of the RPS system.
"There was no impact on the health and safety of the public or plant personnel."
The NRC Resident Inspector has been notified.
