AGREEMENT STATE REPORT - STUCK SOURCE
The following is a summary of information provided by the Arkansas Department of Health via email:
On Thursday, April 17, 2025, at 1309 CDT, the licensee submitted a notification that on Wednesday, April 16, 2025, at 1730 CDT, they had experienced a radiography source failure to retract. The radiation safety officer (RSO) was notified. They arrived onsite at 1930 CDT with retrieval tools and shielding and was able to retract the source into the camera by 2330 CDT. The location was in an isolated area of the Union Pacific rail yard with concrete abutments that afforded shielding of the source. Exposures were limited to 134 millirem to the RSO, 36 millirem to the radiographer, and 11 millirem to the trainee. There was no exposure to the public.
The exposure device is a SPEC 150, serial number 2240, with a SPEC model G-60 source, serial number FL1805, with 33 curies of Ir-192.
The cause was determined to be a kink or crimp in the guide tubing preventing the source from retracting. The investigation is ongoing, and reporting will proceed in accordance with SA-300.

AGREEMENT STATE REPORT - LOST OR STOLEN DENSITY GAUGE
The following report was received by the California Radiation Control Program (the Agency) via email:
"On April 21, 2025, the licensee's radiation safety officer (RSO) contacted the Agency to report the theft of a Troxler 3440 moisture density gauge, serial number 31320. The theft occurred on April 20, 2025, at 2240 PDT at a jobsite in Los Angeles, California. The Los Angeles Police Department was contacted, and a police report was filed. The Troxler gauge contains an 8 mCi cesium-137 sealed source and a 40 mCi americium-241/Be sealed source. The investigation is ongoing."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL
Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. LOSS OF OFF-SITE COMMUNICATION
The following information was provided by the licensee via phone and email:
"On 4/28/2025 at 1026 CDT, the National Weather Service notified Cooper Nuclear Station that the Shubert tower is disabled due to planned maintenance. The planned maintenance will continue from Monday, 4/28/2025 to Wednesday, 4/30/2025. This constitutes a loss of the primary public prompt notification system for greater than 24 hours. The Shubert tower transmitter activates the emergency alert system (EAS)/tone alert radios used for public notification. The backup notification system has been verified to be available for use, if needed, throughout this period. This condition is reportable under 10 CFR 50.72(b)(3)(xiii) as a major loss of the public prompt notification system capability since the backup alert methods do not meet the primary systems design objective.

The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:
The National Weather Service notified the local authorities that the Shubert tower is disabled due to planned maintenance.
