EN Revision Text: CONCURRENT REPORT - EXHAUST FANS SHUT DOWN UNEXPECTEDLY
The following information was provided by the licensee via phone and email:
"This event notification is being made per the requirements of 10CFR70 Appendix A (c) concurrent reports.
"Framatome's Washington State Department of Health Radioactive Air Emissions License requires Framatome to notify WDOH of any time an exhaust fan is shutdown for more than 4 hours.
"On 12/26/2024, at about 1615 PST, the specialty fuels furnace addition exhaust fans shut down due to an electrical breaker tripping.
"No processes were operating and no work was being performed in the building when the fans shut down.
"The power to the main breaker has been shut off and tagged out of service pending investigation into the cause of the breaker trip that shut down the exhaust fans. The main breaker panel will remain shut off until power can be safely restored to it."
The licensee notified the NRC fuel cycle project manager via voicemail.

PART 21 INTERIM REPORT - TORQUE SWITCH FAILURES
The following is a summary of information provided by the licensee via phone and email:
During installation of a replacement torque switch into a SMB-00 actuator at Beaver Valley Nuclear Power Station, technicians reported that the roller subassembly which connects the torque switch to the actuator drive train was loose and easily removed from the torque switch. The condition was identified on a quantity of two torque switches. Investigations by Flowserve indicate that the torque switch assemblies contain a manufacturing error resulting in the torque switch roller pin to be improperly secured in the tripper arm. This condition can potentially result in the roller subassembly becoming detached from the torque switch rendering the torque switch assembly non-functional. However, none of the components were installed in plant equipment.
The extent of condition investigation of the issue is ongoing and the expected date of completion is on or before 2/28/25.

INOPERABILITY OF BOTH TRAINS OF THE CONTROL ROOM EMERGENCY AIR TEMPERATURE CONTROL SYSTEM (CREATCS)
The following information was provided by the licensee via phone and email:
"At 0855 EST on 01/04/2025, it was discovered that both trains of the CREATCS were simultaneously inoperable; therefore, this condition is being reported as an eight-hour, non-emergency notification per 10 CFR 50.72(b)(3)(v)(D).
"At 1408 on 01/04/2025, the 'A' main control room (MCR) chiller was restored to operable following corrective maintenance, and technical specification (TS) limiting condition for operation (LCO) 3.0.3 was exited. The 'B' MCR chiller remains inoperable due to a component outage.
"There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified."
The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:
The 'A' MCR chiller tripped due to low oil level. LCO 3.0.3 was entered in accordance with TS 3.7.11.
The following information was provided by the licensee via phone and email:

TECHNICAL SPECIFICATION VIOLATION
The following information was provided by the licensee via phone:
On January 6, 2025, the licensee reported that it had violated Technical Specification 7.1.3.2 which requires the reactor operator to be present in the control room when the reactor is not secured. The reactor operator left the control room for three minutes to conduct routine maintenance. Upon returning to the control room, the reactor operator discovered the reactor key was still in the console.
The NRC Project Manager was notified.
