The following information was provided by the state of Colorado via email:
"During the morning of 12/17/2021, a portable gauge user noticed their truck had been broken into [in Westminster, CO] and a portable gauge was stolen from the truck. The portable gauge is a Troxler model 3430, serial number 32370, containing 9 mCi cesium-137 and 44 mCi americium-241:beryllium. The stolen gauge has been reported to the local law enforcement."

The following information was provided by Colorado Department of Public Health and Environment (CDPHE) via email:
"On 12/26/24, CDPHE was notified that a member of the public had taken possession of a portable moisture/density gauge. From photos received by CDPHE, the gauge was likely the gauge stolen from the event number above. However, photos did not include the serial number for the gauge, so it is possible the gauge is from EN56084 / NMED item number 220396.
"Prior to interacting with CDPHE, the member of the public learned that the gauge contained radioactive material and abandoned the gauge outside of their property. The gauge was then picked up by an unknown party and the current location of the gauge is unknown.
"Assistance from the Colorado State Patrol has been requested for this event. CDPHE will provide additional updates as we receive them."

The following information was provided by Colorado Department of Public Health and Environment (CDPHE) via email:
"On 1/2/25, CDPHE was notified that the Colorado State Patrol had located the Troxler model 3430 that was originally stolen in 2021 (event number 55664). The portable moisture/density gauge is in the process of being returned to a licensed location.
"CDPHE received confirmation that the gauge referenced in the 12/26/24 notification was for EN55664 and unrelated to EN56084. No further updates are expected regarding EN55664."

AGREEMENT STATE REPORT - DAMAGED SAFETY INSTRUMENT
The following information was provided by the South Carolina Department of Environmental Control (the Department) via email:
"At approximately 2100 EST, on December 24, 2024, the licensee's staff noticed a tear in the Mylar window on one of their Thermo-Fisher FC-185 beta gauges, thus causing the system to be shut down. The system was put back into service at approximately 0000 on December 25, 2024. The licensee's radiation safety officer contacted the Department to report the incident at 1502 on December 25, 2024. The Thermo-Fisher Model TFC-185 contains 1250 mCi of Kr-85. The source model number is a Kr85-4, and the serial number is KA-2197. There were no radiation exposures resulting from the service of the gauge."

CONCURRENT REPORT - EXHUAST FANS SHUT DOWN UNEXPECTEDLY
The following information was provided by the licensee via phone and email:
"This event notification is being made per the requirements of 10CFR70 Appendix A (c) concurrent reports.
"Framatome's Washington State Department of Health Radioactive Air Emissions License requires Framatome to notify WDOH of any time an exhaust fan is shutdown for more than 4 hours.
"On 12/26/2024, at about 1615 PST, the specialty fuels furnace addition exhaust fans shut down due to an electrical breaker tripping.
"No processes were operating and no work was being performed in the building when the fans shut down.
"The power to the main breaker has been shut off and tagged out of service pending investigation into the cause of the breaker trip that shut down the exhaust fans. The main breaker panel will remain shut off until power can be safely restored to it."
The licensee notified the NRC fuel cycle project manager via voicemail.

PART 21 INTERIM REPORT - TORQUE SWITCH FAILURES
The following is a summary of information provided by the licensee via phone and email:
During installation of a replacement torque switch into a SMB-00 actuator at Beaver Valley Nuclear Power Station, technicians reported that the roller subassembly which connects the torque switch to the actuator drive train was loose and easily removed from the torque switch. The condition was identified on a quantity of two torque switches. Investigations by Flowserve indicate that the torque switch assemblies contain a manufacturing error resulting in the torque switch roller pin to be improperly secured in the tripper arm. This condition can potentially result in the roller subassembly becoming detached from the torque switch rendering the torque switch assembly non-functional. However, none of the components were installed in plant equipment.
The extent of condition investigation of the issue is ongoing and the expected date of completion is on or before 2/28/25.
