MOISTURE DENSITY GAUGE DAMAGED BY VEHICLE

The following information was provided by the licensee via phone call:

At 1530 EDT on October 5, 2022, a Troxler gauge Model 3430, containing 8 mCi of Cs-137 and 40 mCi of Am 241, was damaged by a construction vehicle. The sources were in the test position at the time of the event. The unit's source rod was damaged, but the sources were not damaged. The gauge area was quarantined and a repair company was contacted. The repair company provided the licensee a lead lined container for the damaged gauge to be transported back to the vendor. There were no exposures and a leak test is in progress. The repair company took possession of the gauge.

AGREEMENT STATE REPORT - TWO ABANDONED SOURCES

The following information was provided by the MA Department of Public Health (the Agency) via email:

"On 10/05/2022, at 1230 [EDT] , a telephone report was received by the Agency that two discs, each measuring 1.5 inches in diameter and each determined to be containing 0.024 mCi Ra-226 (0.048 mCi total), were found within a load of residential trash picked up in Milton, MA. Each disc measured 6 mR per hour at 3 inches from surface. The discs reportedly did not have any identifying information or markings (no manufacturer name, no date, no serial number) other than 'poison inside'. The discs were placed in secure storage until proper disposal is arranged and completed. The discs are currently presumed to have been inadvertently and unknowingly abandoned by owner. Investigations by the Agency and by Covanta Braintree Transfer Station are ongoing.

The following information was provided by the licensee via e-mail:

"NFS [Nuclear Fuel Services, Inc.] evaluated a degraded Item Relied On For Safety (IROFS) in accordance with the documented Integrated Safety Analysis. During this review, the event was determined to be NON-REPORTABLE as additional controls were available and performance criteria maintained. However, during the detailed review of past performance, when the IROFS that was available in this scenario failed, the redundant IROFS would have been degraded.

"There were no actual radiological or other nuclear safety consequences to the public, workers, or the environment.

"The Senior Resident Inspector and Region II staff were notified."

The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:

The plant was shutdown pending repair or implementation of compensating measures.

EN Revision Text: PART 21 - DEVIATION TO QUALIFIED DESIGN

The following is a summary of information provided by Flowserve - Limitorque via email:

Entergy Waterford 3 informed Flowserve - Limitorque on 10/5/2022 that it discovered that a Peerless 125 volt, 25ft-lb, 56 frame direct current (DC) motor had two fasteners securing the brush holder ring assembly to the motor frame. However, the DC motor assembly used in the qualification test program was assembled with 4 fasteners. Therefore, the use of two fasteners is a deviation to the qualified design. Flowserve is submitting this report as an interim report and is evaluating this deviation to determine whether this condition could potentially affect the safety related function of DC powered Limitorque actuators.

Flowserve is continuing to work with the motor original equipment manufacturer (OEM) to refine the scope of potentially affected motors. Limitorque actuators equipped with Peerless - Winsmith DC electric motors with start torque ratings of 40 ft-lb and larger are not affected by this issue. Limitorque actuators equipped with alternating current (AC) powered electric motors are not affected by this issue.

Flowserve provided an update to notify that the final evaluation of the safety consequence and reportability, primarily involving seismic qualification, will be delayed until 02/24/2023.

Flowserve has completed the evaluation of the deviation described in the initial notification. Flowserve has concluded that the deviation has no substantial impact to the safety function of the component and/or the associated actuator.
