EN Revision Text: AGREEMENT STATE REPORT - POTENTIAL OVEREXPOSURE TO EXTREMITY

The following information was received from the Massachusetts Radiation Control Program (the Agency) via email:

"Licensee reported employee weekly finger ring exposure exceeding 50 rem (67.569 rem for wear period, 7/4/22-7/10/22) a situation reportable within 24 hours per 105 CMR 120.282 (B) (1) (c). The other finger ring dosimeter worn by the subject employee received 31.509 rem exposure for same wear period. Exact cause of exposure not yet known. Licensee investigation ongoing. Licensee, a radiopharmaceutical manufacturer/distributer, stated that subject employee does perform work involved with F-18 radiopharmaceutical manufacturing operations.

"The Agency considers this matter still open."

"On August 17, 2022, An Agency investigation was performed at the PETNET facility. The inspector determined that the root cause of the event was insufficient training of staff. The licensee submitted sufficient corrective actions to prevent recurrence. The Massachusetts Radiation Control Program considers this incident closed."

AGREEMENT STATE REPORT - FIRE EXPOSURE TO RADIATION SOURCE


The following information was provided by the Oklahoma Department of Environmental Quality (DEQ) via email:

"Western Farmers Electric Cooperative (WFEC) currently holds a radiation license (OK-19428-01) for multiple gauges utilized at [the WFEC] Hugo Power Plant. This is a steam plant that operates with coal to generate electricity. A fire was reported at the plant exposing one gauge to high temperatures but did not expose the gauge to direct flames. Based on [the licensee's] phone conversation [with the DEQ] on August 8, 2022, at 1136 [CDT]. The licensee provided this description of the events.

"On August 5, 2022, at 2240 [CDT] smoke was reported in coal bunker D. Staff immediately began procedures to extinguish the fire. Coal bunker D is located directly below coal feeder D and this caused thermal heat around the piping which feeds coal feeder D. A plug detector, which is a RA-226 source, is mounted around this pipe. By 0600 [CDT] on August 6, 2022, the Coal Bunker D was down to 120 degrees [Fahrenheit]. The plant staff notified the WFEC Radiation Safety Officer (RSO), at 0640 [CDT] to check the nuclear gauging device (D FDR Top, serial #SE2562, source amount is 2 mCi).

"Upon arriving at the plant, the RSO surveyed the area around the gauge and determined the area was safe. The gauge had received radiant heat from the fire causing the paint to be removed from the gauge. The survey meter read 0.7mR/hr to 1.0mR/hr around the gauge which is normal. The RSO also conducted an in house leak test checking with the survey meter. The background was 0.1mR/hr and sample was 0.2mR/hr. WFEC is contacting the manufacturer to determine if further inspection is needed."

The licensee believes that no radiation exposure or leaks have occurred. Additional information will be provided by the DEQ as it becomes available.

AGREEMENT STATE REPORT - RADIOPHARMACEUTICAL PACKAGE LOST IN TRANSIT

The following information was received via email:

"The Illinois Emergency Management Agency (the Agency) received notice of a missing radiopharmaceutical package lost in the care of a commercial carrier while in transit from Arlington Heights, IL to Dallas, TX. The package was initially damaged and identified at the carrier's Memphis, TN location on 8/5/22. At that time, it was accounted for and was not reported as lost. As of 8/25/22, the carrier declared the package as lost after attempts to overpack and return to the shipper, GE Healthcare. The carrier reports they lost the overpack with the package inside at their Memphis, TN facility. The 1.0 millicuries of Indium-111 [originally contained in the package] has now decayed beneath 20 microcuries. As a result, this does not represent a significant public safety hazard and there is no indication of intentional theft or diversion. TN program staff have been copied on the notification. All reporting timelines were met. Unless additional information is provided, this matter is considered closed."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
