EN Revision Text: AGREEMENT STATE REPORT - DAMAGE TO GAUGE FROM FIRE

The following event was received from the South Carolina Department of Health [the Department] via email:

"On 3/27/2022, the Department was notified by the licensee's [radiation safety officer] RSO that one of its fixed gauges was damaged during a fire at the plant. The fixed gauge is a TN Technologies Model 5206 containing 200 mCi of Cs-137. The licensee took radiation readings around the gauge and has contacted System Services. Systems Services is arriving on the evening of 3/27/22 to service the gauge. The licensee's radiation readings were 3 mR/hr on the surface of the bottom of the gauge, 250 mR/hr on the top of the gauge. The licensee also took a radiation reading on the walkway three feet above the gauge and the reading was 0.3 mR/hr. The licensee also took a reading at the bottom of the ladder, and it was 0.1 mR/hr. The licensee has barricaded roped off areas on the walkway and at the bottom of the ladder. The gauge is located approximately 12 feet off the floor and is not readily accessible by employees."

EN Revision Text: AGREEMENT STATE REPORT - LOST MASS SPEC-GAS CHROMATOGRAPH

The following information was received from the state of New Jersey Bureau of Environmental Radiation (the Bureau), Radioactive Materials Program (RMP) via email:

"On 3/28/2022 RMP staff was notified via [Radiation Safety Officer] RSO that a mass spec-gas chromatograph with a 15 mCi Ni-63 source (S/N U11650) was sold from their Rahway site without the source being removed. On 10/18/2021, the unit was moved to a surplus warehouse in Kenilworth, NJ. At the time it arrived at the warehouse, the radiation safety label was removed. On 12/30/2021, the unit was sold by the contract service. The RSO will update the Bureau throughout their investigation to find the root cause."


THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. EN Revision Text: UNUSUAL EVENT DECLARED DUE TO A FIRE AT THE 3B REACTOR FEEDWATER PUMP

The following information was provided by the licensee via fax:

"At 1345 CDT, Browns Ferry declared a Notification of Unusual Event due to a fire at the 3B Reactor Feedwater Pump within the Turbine Building which was not extinguished within 15 minutes. Subsequently, the fire was extinguished at 1402 CDT. Unit 3 remains in Mode 1 at approximately 9.5 percent rated thermal power (RTP). Unit 1 and 2 remain at 100 percent RTP and unaffected."

The following additional information was obtained from the licensee in accordance with Headquarters Operations Officers Report Guidance:

The fire began at 1332 CDT. It is believed that the fire was in the oil system of the Feedwater Pump. The fire was extinguished by the on-site fire brigade. No off-site assistance was requested. The Unusual Event was declared under Emergency Action Level HU-4. The licensee notified the NRC Resident Inspector and required State and local government agencies. Unit 3 is currently stable.
