EN Revision Text: AGREEMENT STATE REPORT - MEDICAL EVENT
The following information was received via E-mail:
"On January 26, 2022 the licensee informed the Department (Department of Environmental Protection) of a medical event. It is reportable as per 10 CFR 35.3045(a)(1)(i).
"The Department received notification from a licensee on January 26, 2022 of a medical event involving Yttium-90 TheraSpheres. The entire procedure occurred without incident and the routine contamination survey identified no contamination. However, when the waste was measured following the procedure, the percent of prescribed activity administered was 70.6 percent (administered = 27.72 mCi, prescribed activity = 39.24 mCi). No harm is expected to the patient.
"No further information is available at this time. The Department is currently in contact with the licensee and will update this event as soon as more information is provided."

"On January 25, 2022, at [1430 CST], LDEQ [Louisiana Department of Environmental Quality] received this event notification. The facility involved was the Northeast Louisiana Cancer Institute. In the second quarter of 2021, between 4/14/2021 and 7/14/2021, a technician's Landauer Badge report indicated an exposure of 8407 mrem. The technician worked in a CT and HDR integrated vaults.
"LDEQ has not received any prior information or indication of a radiation overexposure from this facility until this time."

EN Revision Text: CHEMICAL REACTION AND FIRE IN BOTTLE CONTAINING CLEANING MATERIAL
The following information was provided by the licensee via email:
"On January 25, 2022, at approximately 0830 EST, a chemical reaction occurred in a 2-liter bottle of cleanup material. The contents were observed smoldering which then resulted in a small fire inside a process enclosure. The container was damaged in the fire, releasing contents to the enclosure. The NFS Fire Brigade responded and successfully extinguished the fire inside the enclosure upon identification.
"No equipment damage was identified outside of the process enclosure. There were no personnel injuries, exposures, or contamination and no releases to the environment as a result of this event.
"The licensee notified the NRC Resident Inspector."
