The following information was received via E-mail:
"On January 10, 2022, at 1535 CST, the Louisiana Department of Environmental Quality (LDEQ) was notified by the Union Carbide Corporation Radiation Safety Officer, via phone, concerning a handle that had broken off an Ohmart Vega FH-F2-0 nuclear gauge holder (housing). The holder serial number is 20285009. The source serial number is 156OCO. The source activity is 500 mCi of Cs-137. The gauge is in the 'On' position in its present condition."
"This event occurred at the Union Carbide Corporation Taft Plant in Taft, Louisiana."

EN Revision Text: NON-AGREEMENT STATE REPORT - LOST PACKAGES IN TRANSIT
The following is a summary of information received from the licensee via telephonic communication:
On December 10, 2021, 15 packages were scheduled to be delivered by a common carrier to multiple locations in New York. It was then discovered that two packages were missing. The packages contained 44 mCi of Ga-67 and 7.2 mCi of Cu-64, both in liquid form and used for nuclear medicine diagnostic testing. The carrier investigated and searched for the packages but were unable to locate them and they were declared lost on Dec. 17, 2021.
The licensee will notify the Region III office and the New York Department of Health.
THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL
Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
