EN Revision Text: AGREEMENT STATE REPORT - NI-63 SURFACE CONTAMINATION

The following was received from Kentucky Radiation Health Branch (KY RHB) via email:

"At approximately 0950 EDT, the RSO [Radiation Safety Officer] of East Kentucky Power KU [Kentucky Utility] Central Lab called the Kentucky Radiation Health Branch staff to report a Perkin Elmer [gas chromatograph] Model N61000063, s/n 2089, possible leak of the Ni-63 source. A review of the most recent leak test dated 7/29/21, with results of 1.49E-3 microCi, indicated an increase in removable activity when compared to report dated 2/20/21, which demonstrated removable activity of 2.70E-5 microCi.

"The level of activity removed reported to be 1.49E-3 microCi and did not exceed the reporting requirements of 0.005 (5.0E-3) microCi.

"The RSO made arrangements for disposition of materials and will provide that document when completed.

"KY RHB file created for documentation and historical record. These sources had a history of reports as noted in an 8-1-2001 NRC memo."

EN Revision Text: AGREEMENT STATE REPORT - STOLEN XRF ANALYZER

The following report was received from the state of California via email:

"On 08/06/21, California Office of Emergency Services (Cal OES) contacted Radiologic Health Branch (RHB) to report an incident involving loss of a Thermo Fisher Scientific, Niton X fill P 300 series XRF analyzer, Model XLP 300, S/N: 91800. The unit was resourced in May 2018 with 40 milliCi of Cd-109, with current activity of 6.7 milliCi. The XRF unit was taken from one of their employee's vehicle parked at his residence in San Jose, CA. A police report has been filed with the San Jose PD (report # is not available at this time). RHB will be following up with this investigation."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. EN Revision Text: AGREEMENT STATE REPORT - MEDICAL EVENT (INCORRECT DOSAGE)

The following report was received from the Utah Department of Environmental Quality, Division of Waste Management and Radiation Control (the Division) via email:

"The Division was notified of the medical event about 1155 MDT on August 10, 2021, by the licensee's Radiation Safety Officer. At about 0730 a male patient reported to Intermountain Medical Center to have a nuclear medicine scan conducted. The scheduled scan was to use 10 milliCi of Fluorodeoxyglucose (FDG) (F-18). The Technician administered the dosage to the patient and then realized he had administered the wrong dosage. The dosage administered was actually 104 milliCi of FDG which is much greater than the 10 milliCi that was prescribed. The licensee is in the process of notifying the referring physician and the patient of the medical event. The licensee is aware of the 15 day requirement for a written report.

"The Division will investigate this matter and update the record upon completion of the investigation."

A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
