The heat from the pipe | | melted the rubber housing on the crank-out assembly. Could not mechanically | | retract the source. The radiographers isolated the area and kept personnel | | out until AEA Technology could arrive to retrieve the source. There was no | | damage to the exposure device or the source. There was no exposure to | | individuals of the public. The fire was | | put out at 1125 by the fire brigade and no plant personnel were hurt due to | | this incident.
