The doors are normally open doors, | | designed to close automatically when a single temperature sensitive fusible | | link melts. Contrary to the design it was discovered that the installation | | of the doors and associated fusible links may prevent the doors from | | closing. In one scenario it would require two fusible links to melt to get | | the door closure. This condition existed due to Kewaunee's failure to | | properly install the actuation equipment. | | | | Prior to the discovery, the doors have been closed as part of efforts | | involving a previous door failure (Event #37668 and LER 2001-01-00). | | | | The licensee will keep the doors administratively closed until the problems | | are corrected. | | | | The licensee notified the NRC Resident Inspector. | +------------------------------------------------------------------------------+ +------------------------------------------------------------------------------+ |Power Reactor |Event Number: 37769 | +------------------------------------------------------------------------------+ +------------------------------------------------------------------------------+ | FACILITY: FITZPATRICK REGION: 1 |NOTIFICATION DATE: 02/21/2001| | UNIT: [1] [] [] STATE: NY |NOTIFICATION TIME: 17:29[EST]| | RXTYPE: [1] GE-4 |EVENT DATE: 02/21/2001| +------------------------------------------------+EVENT TIME: 15:15[EST]| | NRC NOTIFIED BY: BOB STEIGERWALD |LAST UPDATE DATE: 02/21/2001| | HQ OPS OFFICER: FANGIE JONES +-----------------------------+ +------------------------------------------------+PERSON ORGANIZATION | |EMERGENCY CLASS: N/A |GLENN MEYER R1 | |10 CFR SECTION: |ELINOR ADENSAM NRR | |*UNA 50.72(b)(3)(ii)(B) UNANALYZED COND OP | | | | | | | | | | | +-----+----------+-------+--------+-----------------+--------+-----------------+ |UNIT |SCRAM CODE|RX CRIT|INIT PWR| INIT RX MODE |CURR PWR| CURR RX MODE | +-----+----------+-------+--------+-----------------+--------+-----------------+ |1 N Y 100 Power Operation |100 Power Operation | | | | | | | +------------------------------------------------------------------------------+ EVENT TEXT +------------------------------------------------------------------------------+ | DISCOVERY OF A HATCH PLUGGED THAT SHOULD NOT BE PLUGGED | | | | The licensee was conducting an engineering review of HELB analysis in the | | reactor building which revealed the fact that a hatch was plugged that | | should not be plugged per the analysis. The plug is thought to have been in | | place since about 1983. This questions operability of various safety related | | power supplies. This discovery puts the plant into a 24 hour plant shutdown | | technical specification. The licensee expects to fix the problem prior to | | the 24 hour clock time limit. | | | | The licensee intends to notify the NRC Resident Inspector. | +------------------------------------------------------------------------------+ +------------------------------------------------------------------------------+ |Power Reactor |Event Number: 37770 | +------------------------------------------------------------------------------+ +------------------------------------------------------------------------------+ | FACILITY: SAN ONOFRE REGION: 4 |NOTIFICATION DATE: 02/21/2001| | UNIT: [1] [2] [3] STATE: CA |NOTIFICATION TIME: 18:59[EST]| | RXTYPE: [1] W-3-LP,[2] CE,[3] CE |EVENT DATE: 02/21/2001| +------------------------------------------------+EVENT TIME: 13:00[PST]| | NRC NOTIFIED BY: ANDY BOLYEN |LAST UPDATE DATE: 02/21/2001| | HQ OPS OFFICER: FANGIE JONES +-----------------------------+ +------------------------------------------------+PERSON ORGANIZATION | |EMERGENCY CLASS: N/A |LINDA SMITH R4 | |10 CFR SECTION: | | |*PRE 50.72(b)(2)(xi) OFFSITE NOTIFICATION | | | | | | | | | | | +-----+----------+-------+--------+-----------------+--------+-----------------+ |UNIT |SCRAM CODE|RX CRIT|INIT PWR| INIT RX MODE |CURR PWR| CURR RX MODE | +-----+----------+-------+--------+-----------------+--------+-----------------+ |1 N N 0 Decommissioned |0 Decommissioned | |2 N Y 100 Power Operation |100 Power Operation | |3 N N 0 Hot Standby |0 Hot Standby | +------------------------------------------------------------------------------+ EVENT TEXT +------------------------------------------------------------------------------+ | OFFSITE NOTIFICATION DUE TO DISCOVERY OF DEAD BROWN PELICAN | | | | The licensee made a call to the U. The | | pelican was found at 0800 PST and identified it as a brown pelican, | | protected by the Endangered Species Act, at 1200 PST. | | | | The licensee intends to notify the NRC Resident Inspector. | +------------------------------------------------------------------------------+
