| LOST SAFETY PARAMETER DISPLAY SYSTEM AND THE TECHNICAL SUPPORT CENTER        |
| COMPUTER                                                                     |
|                                                                              |
| Power was lost  to the Safety Parameter Display System (SPDS) and the        |
| Technical Support Center (TSC) computer system at 0904 EDT on 9/12/99.  The  |
| bus supplying power to the battery charger for the SPDS and TSC computer was |
| lost when the supply breaker opened on a ground detection, on the            |
| underground cable supply.  The normal alternate power supply to the battery  |
| charger was previously lost, some time ago, due to another ground on its     |
| underground cable supply.  The licensee made the decision to preserve the    |
| battery and deenergized the battery bus.  Also, with the TSC computer system |
| out-of-service, the control room will serve as the alternate TSC facility.   |
|                                                                              |
| This event is a major loss of emergency  assessment capability and is        |
| reportable.  Troubleshooting and repairs to the electrical distribution      |
| system are in progress.  Also, the licensee is investigating an alternate    |
| methods to power the battery charger.                                        |

| The licensee notified the NRC Resident Inspector.                            |

| 14.1.2.  All emergency diesel generators and their emergency core cooling    |
| systems are fully operable. The electrical grid is stable. The licensee said |
| that they would have to enter Abnormal Operating Procedure 13 two hours      |
| before wind speed increased to 75 mph.  The licensee projects that they have |
| 27 hours until hurricane force winds reach their site.                       |
| The licensee has notified the state of North Carolina and surrounding        |
| counties of entering an unusual event classification.                        |

| conditions as of 0458EDT were wind direction from 240-280 degrees, speed     |

| OFFSITE NOTIFICATION DUE TO OPENING STORM DRAIN BYPASS VALVES                |

| On September 15, 1999, at 0935 EDT, the bypass valves from the site's storm  |
| drain basin directly to the circulating water discharge canal were opened to |
| prevent site flooding (which could cause significant damage to plant         |
| equipment and/or possibly endanger personnel safety) due to the impending    |
| arrival of hurricane Floyd.  Past experience has shown that the installed    |
| pump capacity will not keep up with the rain expected during a hurricane.    |
| The valves were opened prior to hurricane force winds reaching the site to   |
| avoid exposing personnel to the hurricane.   Opening these valves is a       |
| reportable event (to the state of North Carolina) in accordance with the     |
| site National Pollutant Discharge Elimination System permit.                 |

| PATIENT RECEIVED A PLACEBO INSTEAD OF 20 GRAY OF P-32.                       |
|                                                                              |
| A patient signed a consent form to participate in a coronary brachytherapy   |
| trial to evaluate the Guidant Coronary Afterloader.  The patient was         |
| randomized to receive 20 Gray to the proximal left anterior descending       |
| coronary artery with P-32, but the Radiation Oncology physicist programmed   |
| the afterloader to use the placebo source wire instead.  Therefore, the      |
| patient did not receive the prescribed dose of 20 Gray.  The root cause for  |
| the error was that the Radiation Oncologist did not adequately confirm the   |
| written directive prior to treatment.  There was no documentation on the     |
| randomization order to guide the physicist in selecting the proper           |
| treatment, so he relied on his memory, which failed.                         |

| At 1536, the licensee was notified that the National Weather Service had     |
| issued hurricane warnings for North Carolina and Virginia, with hurricane    |
| force winds extending 140 miles outward from the center of Hurricane Floyd.  |
| The licensee considers that the arrival of hurricane force winds within 500  |
| nautical miles (NM) of Indian Point, concurrent with hurricane warnings is   |
| imminent. This notification is being made in accordance with Technical       |
| Specification 3.14.a and Abnormal Operating Instruction 28.07. The NRC       |
| resident inspector will be informed of this notification by the licensee.    |

| Shutdown of Unit 2 commenced at 1906 EDT, and Unit 1 began shutting down at  |

| REACTOR MANUALLY SCRAMMED FROM 52% REACTOR POWER DUE TO AN UNCONTROLLABLE    |
| LOSS OF ELECTROHYDRAULIC CONTROL (EHC) FLUID.                                |

| At approximately 1750 on 09/15/99, failure of an EHC system pressure sensing |
| line resulted in an uncontrollable loss of EHC fluid.  Attempts to maintain  |
| EHC reservoir level failed, and a 1825 hours, the reactor was manually       |
| scrammed from 52% reactor power in preparation for removing the EHC system   |
| from service.  Reactor power was initially at 100% and it was reduced to 52% |
| before the reactor was manually scrammed.  The scram resulted in a low       |
| reactor vessel water level, causing an automatic isolation of Primary        |
| Containment Isolation System (PCIS) groups 2 (Drywell isolations), 3         |
| (Reactor Water Cleanup trip/isolation), &8 (transversing incore probe system |
| isolation).  EOI-1  was entered due to low reactor vessel water level        |
| {decreased to negative 28.5", Top of Active Fuel (TAF) is negative 162"} and |
| to control reactor pressure with the loss of the EHC system.  No Main Steam  |
| Safety Relief Valves (MSRVs) were actuated during this event.  All control   |
| rods fully inserted into the core.                                           |
|                                                                              |
| The licensee started that restart is projected for 1600 hours on 09/16/99.   |
|                                                                              |
|                                                                              |
| NRC Region 2 Section Chief (Paul Fredrickson) was notified of this event by  |
| the licensee.                                                                |

| PARTIAL LOSS OF EMERGENCY SIREN CAPABILITY DURING HURRICANE FLOYD            |

| "[Operations was] informed by Brunswick Plant Emergency Preparedness         |
| personnel that 13 emergency warning offsite sirens have failed as a result   |
| of Hurricane Floyd.                                                          |
|                                                                              |
| "[The initial safety significance evaluation characterized the significance  |
| of the loss of the sirens as] minimal.   Backup alert measures are in place. |
| Local agencies have been notified.                                           |
|                                                                              |
| "[Corrective action(s) include a] complete investigation into the cause of   |
| the failure and complete required repairs."                                  |
|                                                                              |
| The thirteen [13] sirens are located primarily on the beach and on Bald Head |
| Island and Boiling Spring Lake  in Brunswick county.  There are an           |
| additional seventeen [17] operable sirens currently available in Brunswick   |
| county and five [5] more in New Hanover county.  The loss of the 13 sirens   |
| was related to loss of power as detected by an external operability check    |
| performed by a computer.  The licensee will not initiate repairs until the   |
| storm passes.                                                                |

| The licensee informed state/local agencies and the NRC Resident Inspector.   |

| "The Plant was operating at 100% power when 22 Warning Sirens became         |
| inoperable due to loss of power. The counties of Chatham, Harnett, and Wake  |
| are each affected. Wind speeds in these areas are higher than normal due to  |
| the effects of hurricane Floyd. One additional Warning Siren in Wake County  |
| was previously inoperable due to failing its periodic test. Repairs will be  |
| made when wind speed subsides."                                              |
|                                                                              |
| Harris has a total of 81 sirens of which 22 represents slightly greater than |
| 25%.  Residents living within 5 miles of the site have tone alert radios as  |
| a backup.  The licensee informed the NRC resident inspector and will inform  |
| state/local agencies.                                                        |

| - Secondary containment integrity lost due to a hatch cover being removed    |
| during maintenance activities -                                              |
|                                                                              |
| At 0115 on 09/16/99. Limerick Generating Station Unit 2 experienced a loss   |
| of secondary containment integrity.  At the time of the event. the Standby   |
| Gas Treatment (SBGT) System was in service.  The Reactor Enclosure HVAC      |
| System was not in service due to maintenance activities. The SBGT System was |
| unable to maintain reactor enclosure pressure greater than 0.25 inches of    |
| vacuum water gauge per Technical Specifications due to a hatch cover being   |
| removed for maintenance inspection activities.  This event could have        |
| prevented the fulfillment of the safety function of structures or systems    |
| that are needed to control the release of radioactive materials or mitigate  |
| the consequences of an accident.  The hatch cover was reinstalled properly   |
| by 0126 and the SBGT System was able to draw a vacuum on the reactor         |
| enclosure area greater than 0.25 inches of vacuum water gauge.               |
