| SALT SERVICE WATER SYSTEM EXCEEDED MAXIMUM DESIGN TEMPERATURE |
| |
| The 'A' train of the salt service water system was declared inoperable when |
| the intake structure temperature exceeded the maximum design temperature of |
| 105�F. The highest actual temperature reached was 106�F, which occurred for |
| approximately 15 minutes, until operators were able to reduce the |
| temperature by realigning the salt service water system. The current intake |
| structure temperature is 100�F. |

| The NRC resident inspector has been informed of this event by the licensee. |

| After further evaluation based on NUREG 1022, Revision 1, the licensee |
| concluded that this event was not reportable. The temperature exceeded the |
| design temperature for only a short period {well within the limiting |
| condition for operation (LCO) time frame}. Only one side of the system was |
| affected, and this was not a common mode type of event. The event was |
| covered under the LCO, and the LCO actions were met. Therefore, the |
| licensee is retracting this event notification. |

| The licensee plans to notify the NRC resident inspector. The NRC operations |
| officer notified the R1DO (Eselgroth). |

| UNPLANNED CONTAMINATION IN AN ARMS ROOM AT FORT DRUM (A U.S. ARMY FACILITY) |
| LOCATED IN WATERTOWN, NEW YORK |
| |
| At 1400 EDT on 08/12/99, individuals located in an arms room were performing |
| maintenance on an M224 mortar range indicator when one of the four sources |
| broke causing an unplanned contamination. The sources each contained 0.8 Ci |
| of tritium. Two individuals were close by, and two bioassays were |
| performed. The bioassay results are currently pending. However, the |
| following wipe test results were received today (08/16/99): |

| 6,300,000 dpm at the site of the broken source, |
| 1,358,000 dpm on the mortar housing, |
| 31,000 dpm on the mortar, |
| 74,000 dpm on an adjacent table top, |
| 74,000 dpm (highest reading) on a wall, and |
| 34,000 - 35,000 dpm on four walls. |
| |
| It was also reported that the maintenance activities should have occurred in |
| a maintenance shop rather than in the arms room (which is used for storage). |
| As a result, there may be additional items with contamination. The arms |
| room has been cordoned off. |

| The licensee reported that the emergency notification system line was out of |
| service as well as 14 of 26 evacuation sirens in York County and 2 of 35 |
| evacuation sirens in Lancaster County. |

| LOSS OF EMERGENCY SIRENS IN HARNETT COUNTY DUE TO A LIGHTNING STRIKE |
| |
| The following text is a portion of a facsimile received from the licensee: |
| |
| "All sirens in Harnett County were lost from 1600 to 1730 on 08/14/99 ... |
| due to a lightning strike [that took out a 911 repeater]. The backup |
| repeater was placed in service at 1730 [on 08/14/99]. The primary repeater |
| was placed in service at 1200 on 08/16/99. The backup repeater is not |
| tested routinely, and it is unknown if it would have actuated the sirens |
| from 1730 [on] 08/14/99 until 1200 [on] 08/16/99. This feature will be |
| tested tomorrow to determine if the sirens were operable on the backup |
| repeater." |
| |
| The licensee stated that there are a total of 81 sirens associated with the |
| Harris facility and that all 7 sirens in Harnett County were lost. This |
| event was determined to be reportable at 2040 on 08/16/99. |
| |
| The licensee plans to notify the NRC resident inspector. |
