| FIRE DAMAGE TO EQUIPMENT CONTAINING RADIOACTIVE MATERIAL |
| |
| On Wednesday December 9, 1998, at 0610 EST a motor fire occurred in the |
| X-326 process building on cell 25-7-2. The fire was put out by the plant |
| fire department. The fire involved lubricating oil in an electrical motor |
| that drives compressors. The fire was limited to a single housing |
| containing the affected equipment. However the fire spread to additional |
| motors and equipment within the cell housing. Damage to the equipment and |
| piping caused the primary containment to be breached. |

| At 2245 EST the Plant Shift Superintendent determined that this event was |
| reportable under 10CFR76.120(c)(4). The fire damaged the radioactive |
| material container (process gas piping) and the equipment contained |
| radioactive material greater than 5 ALls (5 microcuries or 2.2 grams |
| U-235). |

| The certificate holder notified the NRC Resident Inspector, the local DOE |
| representative, the state of Ohio, and issued a press release. |

| EMERGENCY CLASSIFICATION. THE DECLARATION OF AN ALERT WOULD HAVE BEEN |
| REPORTABLE IN ACCORDANCE WITH 10CFR76.120(a)(4), BUT WAS NOT REPORTED AT THE |

| RADIOACTIVE THORIUM MAGNESIUM USAF AIRCRAFT PARTS FOUND IN SCRAPYARD |
| |
| At 0800 CST on 11/03/98, a C&C Alloy Company representative notified the |
| Tinker Air Force Base (TAFB) Bioenvironmental Engineering Office (BEO) that |
| aircraft parts they had purchased from the TAFB Defense Reutilization |
| Management Office may be radioactive. The BEO sent representatives to the |
| C&C Alloy scrapyard in Oklahoma City to survey a crate of scrap metal which |
| contained radioactive parts. Radiation levels of the scrap metal were |
| reading twice as much as background levels. The metal had been cut up into |
| small pieces and is believed to be magnesium thorium panels. No personnel |
| had been exposed to respirable particles during the cutting process. The |
| crate of material has been returned to TAFB for proper disposal. The BEO |
| plans to establish by 01/15/99 a procedure to survey scrap metal before it |
| leaves TAFB. The BEO plans to notify Oklahoma authorities and request that |
| the Air Force Health Physics Branch (Brooks Air Force Base, San Antonio, TX) |
| provide a radiation survey team if C&C Alloy requests that a survey of their |
| scrapyard be performed. |

| 12/16/98. |

| C&C ALLOY RETURNED ALL RADIOACTIVE MATERIAL TO THE AIR FORCE AND DECLINED |
| THE RADIATION SURVEY. A GATE MONITOR WAS INSTALLED FOR THE DEFENSE |
| REUTILIZATION MANAGEMENT OFFICE(DRMO) TO MONITOR ALL SCRAP METAL, AND PRIOR |
| TO ITS INSTALLATION, SCRAP METAL WAS SURVEYED WITH HAND-HELD RADIATION |
| DETECTORS. PERSONNEL WILL BE EDUCATED ON THE USE OF THE GATE MONITOR. |

| THE METAL WAS IDENTIFIED AS COMING FROM A T-53 GAS TURBINE ENGINE, POSSIBLY |
| USED IN THE F MODEL UN-1 HELICOPTER. THE MATERIAL WAS MANUFACTURED BY |
| ALLIED SIGNAL AEROSPACE AND WAS IDENTIFIED AS CONTAINING 2 TO 3% BY WEIGHT |
| OF THORIUM 232. THIS ALLOY MEETS THE EXEMPT REQUIREMENTS OF |
| 10CFR40.13(c)(4). |

| DISCOVERY OF A MISSING CHEMICAL AGENT MONITOR CONTAINING 15 mCi OF NICKEL-63 |
| FROM SHAW AIR FORCE BASE LOCATED IN SUMTER, SOUTH CAROLINA |
| |
| At 0938 on 05/07/99, Shaw Air Force Base (AFB) located in Sumter, South |
| Carolina, notified the U.S. Air Force Radioisotope Committee located at |
| Bolling AFB located in Washington D.C. of a missing chemical agent monitor |
| (CAM) containing a 15-mCi nickel-63 source. |
| |
| On 04/27/99, the base radiation safety officer (RSO) at Shaw AFB conducted |
| an inventory of CAMs located in building #1981. The correct number of CAMs |
| were there, but one had the wrong serial number. Examination of previous |
| inventories shows one CAM with a different serial number. The building had |
| been searched with no success. The 20CES/CEX RSO has no explanation for the |
| different serial number on one CAM or where the correct CAM may be located. |
| |
| |
| It is suspected that the CAM may have been exchanged with another Air Force |
| Unit or may have been sent off for calibration. At this time, the U.S. Air |
| Force Radioisotope Committee believes that the CAM is still in Air Force |
| possession. Past shipping records will be researched, and calibration labs |
| that serve Shaw AFB will be notified. The completion time for these actions |
| is currently estimated to be 06/07/99. |

| THE SITE REPORTED THAT AT 1345, OPERATIONS PERSONNEL DISCOVERED STANDING |
| WATER UNDER AUTOCLAVE #6 IN THE X-343, FEED AND SAMPLING FACILITY. STANDING |
| WATER IS AN INDICATION THAT A DRAIN UNDER THE AUTOCLAVE WAS PLUGGED. THE |
| PLUGGED DRAIN IS NOT THE AUTOCLAVE DRAIN, BUT THE CONTAINMENT PIT UNDER THE |
| AUTOCLAVE. THIS CONDITION IS ANALYZED IN THE NUCLEAR CRITICALITY SAFETY |
| EVALUATION, NCSE-0343_002.001 CONTINGENCY EVENT 6.3a, WHICH STATES IN PART, |
| "THE AUTOCLAVES ARE MOUNTED INSIDE LARGE CONCRETE PITS, WHICH ARE NORMALLY |
| DRY AND ARE PREVENTED FROM ACCUMULATING WATER FROM ANY SOURCE BY A 4 INCH |
| DRAIN." |

| LOSS OF CRITICALITY ACCIDENT ALARM SYSTEM DURING MAINTENANCE |
| |
| "At 1633 CDT on 6/10/99, electricians were performing scheduled maintenance |
| on the C-400 building criticality accident alarm system (CAAS). During this |
| maintenance activity, circuit breaker EP1 in C-400 was opened resulting in a |
| system trouble alarm on the C-409 building 'AE' CAAS cluster. TSR LCO |
| actions were implemented for the C-409 facility due to the unknown cause of |
| the trouble alarm. After further review of this maintenance activity, there |
| is evidence that control power for the CAAS building horns in C-409 and |
| C-720-M was lost for approximately four hours during the subject maintenance |
| evolution. CAAS detectability and C-409 local CAAS horn operability were |
| not affected by the loss of control power. |

| 10CFR76.120(c)(2) as an event in which equipment is disabled or fails to |
| function as designed. |

| POSITIONED(i.e. CLOSED OR TRIPPED) BASED ON PLANT CONDITION OF MODE 4. THE |
| CAUSE APPEARS TO BE A PROCEDURAL DEFICIENCY, WHICH WILL BE CORRECTED WITH A |
| PROCEDURE CHANGE. |

| THE PLANT HAD A MANUAL REACTOR TRIP FROM 100% POWER. |
| |
| A MANUAL REACTOR TRIP WAS INITIATED BECAUSE OF A LOSS OF CONDENSER VACUUM |
| (CAUSE UNKNOWN AND BEING INVESTIGATED). ALL RODS FULLY INSERTED, NO ECCS |
| INJECTION INITIATED AND NO SAFETY OR RELIEF VALVES LIFTED. AUXILIARY |
| FEEDWATER STARTED AS EXPECTED FOLLOWING THE REACTOR TRIP AND THEY ARE USING |
| THE STEAM GENERATORS VIA THE STEAM DUMPS AS A HEAT SINK. ALL SYSTEMS |
| OPERATED SATISFACTORILY FOLLOWING THE TRIP WITH THE EXCEPTION OF THE M-35 |
| INTERMEDIATE RANGE CHANNEL, WHICH FAILED LOW. THE CAUSE OF THIS FAILURE IS |
| ALSO BEING INVESTIGATED. |
