| STOLEN TROXLER MOISTURE/DENSITY GAUGE                                        |

| On 12/29/98, the licensee discovered that a Troxler Model 3440 soil          |
| moisture/density gauge (Serial #24596) had been removed from its case, which |
| was located in the back of a pickup truck.  The gauge contains one 8-mCi     |
| Cs-137 source, and one 40-mCi Am-241/Be source.   When last seen on          |
| 12/23/98, the gauge had been locked in its case, and the case had been       |
| chained to the back of the pickup.  A state investigator from the Miami      |
| office has been dispatched to the scene, and the Metro-Dade police are       |
| investigating the theft.                                                     |

| A fax from the Florida Department of Health Environmental Radiation Control  |
| reported that a person, unknown, walked up to construction site personnel,   |
| dropped off the gauge and left.  Metro Dade Hazmat took possession of the    |
| gauge. A survey showed normal readings.  The owner has been contacted and    |
| will take possession of the gauge.  A leak test will be performed and        |
| results passed to the state office.  Metro Dade police is investigating.     |

| COMMON FAILURE EFFECTING BOTH TRAINS OF SHUTDOWN COOLING                     |
|                                                                              |
| "On 3/9/99, at 0221 it was discovered that the Unit 1 shutdown cooling       |
| valves HV-51-1F015A, HV-51-1F015B, and HV-51-1F08 could not be opened, if    |
| plant conditions required, due to a failure in the isolation logic circuit   |
| connectors.                                                                  |
|                                                                              |
| "This failure affected both shutdown cooling return valves and a common      |
| suction valve which results in one failure effecting both trains of shutdown |
| cooling.  The identified deficiency is being planned for repairs.  This      |
| condition, of a single failure disabling multiple trains of a system, is     |
| being reported pursuant to 50.72(b)(2)(iii)."                                |
|                                                                              |
| This deficiency was discovered during routine maintenance.  These valves     |
| could be opened manually using existing plant procedures if it were          |
| necessary to initiate shutdown cooling.                                      |
|                                                                              |
| The licensee will notify the NRC resident inspector.                         |

|                                                                              |
| "This is a retraction of Event # 35451: Common Failure Effecting Both Trains |
| of Shutdown Cooling                                                          |
|                                                                              |
| "During normal operator rounds in the Auxiliary Equipment room, an operator  |
| noticed an extinguished NSSSS indicator light. An attempt to reset that      |
| portion of the logic circuitry failed. Troubleshooting revealed that there   |
| was an open connection across a Grayboot electrical connector. The connector |
| was replaced and the system was returned to normal. The Residual Heat        |
| Removal (RHR) - Shutdown Cooling Mode (SCM) was not in demand at any time    |
| during the incident. Had the failure existed during a loss of coolant event, |
| high drywell pressure would have prevented the common suction valve for both |
| loops of RHR-SCM from initially being used.                                  |
|                                                                              |
| "A lab analysis of the failed electrical Grayboot connector revealed an      |
| over-crimping of the connector during initial installation. The failure was  |
| attributable to improperly setting the crimping tool from its initial        |
| calibration setting prior to performing the final crimp. Six other Grayboot  |
| connectors from various other Limerick Unit 1 and Unit 2 systems were        |
| removed and analyzed in the lab. No similar incidences of over-crimping were |
| found, therefore the identified condition is considered not to be a generic  |
| concern.                                                                     |
|                                                                              |
| "A licensing basis review for shutdown cooling was performed. Updated Final  |
| Safety Analysis Report (UFSAR) section 7.4.1.3.1.1 states that '...The       |
| system can use a preferred method to accomplish its design objectives by     |
| directly extracting reactor vessel water from the reactor vessel via the     |
| recirculation loop and routing it to a heat exchanger and back to the        |
| vessel. An alternate method is to indirectly extract the water via relief    |
| valve discharge lines to the suppression pool and route the suppression pool |
| water to the heat exchanger and back to the vessel.  The redundancy and      |
| diversity of RHR-SCM is also discussed in UFSAR Section 7.4.1.3.3.5. Failure |
| of RHR - SCM is analyzed in UFSAR Section 15.2.9. Also, UFSAR Section        |
| 15.9.2.4 discusses alternate path availability to accomplish the shutdown    |
| cooling function.                                                            |
|                                                                              |
| "In addition, plant procedures exist to respond to a Loss of Shutdown        |
| Cooling. The Shutdown Cooling system is initiated by Off-Normal procedure    |
| ON-I 21, loss of Shutdown Cooling. Also, the other mechanism for SCM was     |
| available and is also procedurally governed by S41 .7.B  Use of SRV's and    |
| Suppression Pool Cooling as an Alternate Shutdown Cooling Method.            |
|                                                                              |
| "Based on our review of the licensing basis for shutdown cooling, we         |
| determined that the Shutdown Cooling System was not rendered incapable of    |
| performing its design safety function."                                      |
|                                                                              |
| The licensee will inform the NRC resident inspector of this retraction.      |

| AGREEMENT STATE REPORT - LOST DELIVERY CASES                                 |
|                                                                              |
| NC Division of Radiation Protection was informed April 8, 1999, by Syncor    |
| International Corporation that one of their Nuclear Pharmacies lost control  |
| of five delivery cases containing Limited Quantity Shipments (LQS) returned  |
| from customers. The pharmacy is located in Charlotte, NC (Radioactive        |
| License No. 060-0794-1). On October 30, 1998, the delivery cases were        |
| inadvertently left within the trunk of a delivery vehicle which had been     |
| rendered inoperable in an accident. The car was transferred through a series |
| of wrecker and garage services (without a set of keys), eventually being     |
| sold to a resident of TN. The new owner purchased the vehicle on April 2,    |
| 1999,  in SC, returned it to TN and had keys made. The delivery cases were   |
| discovered April 3, 1999, when the owner opened the trunk for the first      |
| time. The owner notified TN authorities who were able to determine the       |
| origin of the delivery by paper work located with the cases. TN authorities  |
| contacted Syncor International Corporation which dispatched trained          |
| employees to recover the delivery cases. Surveys performed at the time of    |
| recovery indicated no exposure rates above background or indications of      |
| contamination.                                                               |

| Conservation Div. Of Radiological Health SC Agreement State Program and      |

| The following information was received via email from the State of           |
| Illinois:                                                                    |
|                                                                              |
| "On April 7, 1999, Medi-Physics Pharmacy (IL-01052-01) in Chicago reported   |
| that they received a package with removable contamination of 265,000 cpm.    |
| The package was picked up and transported by the pharmacy's driver from West |
| Suburban Hospital in Oak Park, IL (IL-01520-01).  The Illinois Department of |
| Nuclear Safety is conducting an investigation at the shipper's facility in   |
| Oak Park into the cause of the event.  The Illinois equivalent to 20.1906(d) |
| is 32 Ill. Adm. Code 340.960(d).  The contaminant was Tc-99m."               |

| CONTROL ROOM VENTILATION INTAKES ISOLATED                                    |
|                                                                              |
| The Unit 2 Control Room Ventilation (VC) intake was isolated when valve      |
| 2VC6A automatically closed due to a (suspected) invalid high chlorine        |
| signal. During troubleshooting activities, a maintenance technician          |
| inadvertently closed valve 1VC5B (the Unit 1 VC intake) when he depressed    |
| the associated chlorine detector reset button.  (The technician mistakenly   |
| thought that the VC intake for Unit 1 was isolated.)  Control room operators |
| immediately discovered the error and reopened valve 1VC5B.  As a result of   |
| this event, both units entered Technical Specification 3.0.3 for             |
| approximately one minute.  The NRC resident inspector will be informed of    |
| this notification by the licensee.                                           |

| HPCI TEMPORARILY INOPERABLE                                                  |

| System Affected: During performance of regularly scheduled Instrumentation   |
| surveillance on Unit 3 HPCI Steam Line Space High Temperature switches (in   |
| accordance with plant procedure 3-SR-3.3.6.1(3D)) personnel noticed that a   |
| relay not tested by procedure was chattering.  The affected relay            |
| 3-RLY-073-23A-K16 was adjacent to a relay 3-RLY-073-23A-K8 which was being   |
| tested by the surveillance.  While troubleshooting the cause of the          |
| chattering relay,  a loose wire was found in the power supply pathway for    |
| the HPCI logic circuit.  This loose wire had the potential to prevent        |
| adequate voltage being supplied to the initiation and isolation logic for    |
| the Unit 3 HPCI system.                                                      |
|                                                                              |
| Actions: The loose wire was retightened.                                     |
|                                                                              |
| Reportability: The potential for inadequate voltage being available for the  |
| HPCI logic circuit resulted in a conservative decision to declare the HPCI   |
| system inoperable. In accordance with plant procedure SPP 3.5, code of       |
| Federal Regulations 10CFR50.72/73 and NUREG 1022 Rev 1 this is reportable as |
| a 4 Hour Non-Emergency report due to any event or condition that alone could |
| have prevented the Fulfillment of the safely function of structures or       |
| systems that are needed to   (d) Mitigate the consequences of an accident.'  |
| This criteria is specified in 10CFR50.72.b.2.iii as a 4 hour report and in   |
| 10CFR50.73.a.2.v as a 30 day written report.                                 |
|                                                                              |
| The licensee notified the NRC resident inspector.                            |
