| - EMERGENCY OPERATIONS FACILITY INOPERABLE DUE TO TWO INOPERABLE AIR         |
| COMPRESSORS -                                                                |

| THE LICENSEE DECLARED BOTH AIR COMPRESSORS AT THE ENTERGY SERVICES CENTER    |
| INOPERABLE WHEN THEY FAILED TO START DURING A TEST.  THESE COMPRESSORS       |
| SUPPLY AIR NECESSARY TO ISOLATE THE EMERGENCY OPERATIONS FACILITY (EOF),     |
| OPERATE THE HVAC SYSTEMS, AND START THE BACKUP GENERATOR.  ONE UNIT HAS A    |
| FAULTY COMPRESSOR AND THE OTHER HAS A FAULTY CIRCUIT BREAKER.                |
|                                                                              |
| THE LICENSEE PLANS TO SWAP CIRCUIT BREAKERS AND RESTORE ONE UNIT TO OPERABLE |
| STATUS.  THE LICENSEE DECLARED THE EOF INOPERABLE.                           |

| THE LICENSEE HAS NOTIFIED STATE AND LOCAL OFFICIALS, LICENSEE EMERGENCY      |
| RESPONSE PERSONNEL, AND THE NRC RESIDENT INSPECTOR THAT THE EOF IS           |
| INOPERABLE AND TO RESPOND TO THE BACKUP EOF LOCATED AT THE BAXTER WILSON     |
| POWER STATION IN VICKSBURG, MS, APPROXIMATELY 30 MILES FROM THE SITE, IF THE |

| - AGREEMENT STATE REPORT - TWO LOST PORTABLE NUCLEAR SOIL MOISTURE DENSITY   |
| GAUGES -                                                                     |
|                                                                              |
| At 1415 on 03/10/99, during a Florida state inspection of J & M Testing,     |
| Inc., two portable nuclear soil moisture density gauges were determined to   |
| be missing.  These are old gauges that were not being used and were last     |
| seen during an inventory taken on 12/28/98.   The Chipley, FL police         |
| department has been notified.                                                |
|                                                                              |
| Specifications of the gauges are as follows:                                 |

| AT 1507 CDT ON 03/23/99, DURING A 10CFR50, APPENDIX R REBASELINE REVIEW, THE |
| LICENSEE DISCOVERED THAT CABLE #ZP2I483F FOR RED CHANNEL STEAM GENERATOR 'B' |
| PRESSURE TRANSMITTER #2PT-00483 ROUTED THROUGH A FIRE ZONE IS NOT PROPERLY   |
| WRAPPED WITH FIRE WRAP.   THUS, THE RED CHANNEL STEAM GENERATOR 'B' PRESSURE |
| TRANSMITTER WOULD NOT BE AVAILABLE, AS EXPECTED, DURING A FIRE IN THIS       |
| ZONE.                                                                        |

| A FOUR HOUR ROVING FIRE WATCH ALREADY EXISTS FOR THIS FIRE ZONE FOR OTHER    |
| REASONS AND WILL REMAIN IN EFFECT UNTIL THIS PROBLEM IS CORRECTED.           |

| THIS PROBLEM IS APPLICABLE TO UNIT 2 ONLY AND HAS NO EFFECT ON UNIT 1.       |
