| 24-HOUR REPORT REQUIRED BY PLANT OPERATING LICENSE DUE TO VIOLATION OF       |
| TECHNICAL SPECIFICATIONS                                                     |
|                                                                              |
| The licensee has discovered that a solenoid-operated isolation valve on a    |
| 3/8" diameter sample line from the secondary containment was excluded from   |
| the plant technical specifications (TS), and, consequently, it was not       |
| subject to surveillances required by the TS.  The valve is located on a      |
| secondary containment atmosphere sample line which connects to the           |
| post-accident sampling system.  The licensee has verified that the valve is  |
| closed.                                                                      |

| The NRC resident inspector has been informed of this event by the licensee.  |

| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "During the extent of condition investigation of Condition Report 98-2142,   |
| it was identified that instrument vent and drain valves associated with the  |
| reactor core isolation cooling system (E51) and the annulus exhaust gas      |
| treatment system (M15) could fit the definition of secondary containment     |
| isolation valves as discussed in Technical Specification Bases 3.6.4.2.      |
| However, these valves are not surveilled and checked closed every 31 days as |
| required by Surveillance Requirement 3.6.4.2.1.  There are 12 valves (3/8")  |
| associated with E51, and 8 valves (1/4") associated with M15."               |

| The licensee plans to continue its evaluation and has informed the NRC       |
| Resident Inspector.  The NRC Operations Officer notified the R3DO (Gavula).  |

| LOSS OF HALF OF THE OFFSITE EMERGENCY SIRENS DUE TO FREEZING RAIN            |
|                                                                              |
| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "10CFR50.72(b)(1)(v) requires that a facility notify the NRC Operations      |
| Center during any event that results in a major loss of emergency offsite    |
| response capability.  Due to weather conditions causing area power outages,  |
| a system test of the emergency sirens was conducted.  During this test, it   |
| was discovered that 22 out of 44 emergency sirens were not functioning.      |
| Therefore, Clinton Power Station is making a notification pursuant to        |
| 10CFR50.72(b)(1)(v)."  The licensee's current site policy is to make a       |
| notification of this type if less than 90% of the emergency sirens are       |
| available.                                                                   |

| The licensee notified the NRC resident inspector.                            |

| As of 0930 CST, the plant restored power to all but four emergency sirens.   |
|                                                                              |

| LOSS OF EMERGENCY NOTIFICATION SYSTEM (ENS) AND COMMERCIAL TELEPHONES        |
|                                                                              |
| At 0300, it was discovered that the FTS-2000 ENS telephone line and the      |
| offsite commercial telephone lines were out of service. The cause of this    |
| telephone outage is unknown at this time.  However, the licensee stated that |
| there does not seem to be a trunk line problem.  The licensee has contacted  |
| Ameritech and a local telephone representative.  Offsite communications are  |
| available via the use of cellular telephones and base radios to the Michigan |
| State Police and to the licensee's power control (load dispatcher).  Outside |
| calls can be made from those locations if it is necessary for the licensee   |
| to relay information.                                                        |

| LICENSEE REPORTED THAT NUMEROUS PATIENTS RECEIVED LESS THAN THE PRESCRIBED   |
| Sr-90 DOSE.                                                                  |

| IN 1998, TWELVE (12) PATIENTS WERE DIAGNOSED AND TREATED FOR PTERYGIUM, A    |
| SUPERFICIAL GROWTH ON THE LENS OF THE EYE,  ELEVEN HUNDRED (1,100) RADS WAS  |
| PRESCRIBED FOR EACH; HOWEVER, DUE TO THE LICENSEE NOT APPLYING A DECAY       |
| FACTOR TO THE STRONTIUM-90 SOURCE, PATIENTS RECEIVED APPROXIMATELY 600 RADS. |
| THE TWO (2) EYE APPLICATORS (SERIAL #s 0302 AND 431 - EACH 100 mCi Sr-90)    |
| WERE  RECEIVED FROM NEW ENGLAND NUCLEAR IN MARCH 1979.  THE DISCREPANCY IN   |
| THE PRESCRIBED VERSUS ADMINISTERED DOSE WAS DISCOVERED DURING AN ONSITE      |
| INSPECTION  BY THE NRC ON 01/13/99.  THE LICENSEE IS CONTINUING ITS          |

| AUTOMATIC REACTOR TRIP ON TRIP OF TWO OF THREE REACTOR COOLANT PUMPS AND     |
| LOSS OF TWO 6.9-kV BUSES                                                     |
|                                                                              |
| During relay calibration on the 'A' 6.9-kV non-safety bus, the bus was       |
| unexpectedly deenergized.  The 'C' 6.9-kV bus is a swing bus and was tied to |
| the 'A' 6.9-kV bus, which caused it to be deenergized also.  This caused the |
| loss of two of three reactor coolant pumps, and in turn, caused an automatic |
| reactor trip from 100% power.  All systems functioned as expected, all rods  |
| fully inserted, and auxiliary feedwater started on low steam generator level |
| as expected.  All safety systems are operable except the 'A' containment     |
| hydrogen analyzer which is out of service for planned maintenance.           |
|                                                                              |
| The exact cause of the loss of the 6.9-kV buses is currently under           |
| investigation.                                                               |
|                                                                              |
| The licensee notified the NRC Resident Inspector, the State of North         |
| Carolina, and the local county.                                              |

| MINUTES.  OFFSITE FIRE ASSISTANCE WAS REQUESTED FROM THE SCRIBA FIRE         |
| DEPARTMENT.  ALL PERSONNEL ARE ACCOUNTED FOR WITH ONE KNOWN MINOR INJURY.    |

| THE LICENSEE IS EVACUATING PERSONNEL FROM ITS BUILDINGS LOCATED ON THE EAST  |
| SIDE OF THE SITE OUTSIDE THE PROTECTED AREA FENCE.  THESE BUILDINGS WERE     |
| APPROXIMATELY SEVERAL HUNDRED YARDS FROM THE FIRE SITE.  THIS ACTION WAS     |
| RECOMMENDED BY A LOCAL OFFICIAL BASED ON THE FIRE AT THE FITZPATRICK SITE    |
| WHICH BORDERS NINE MILE POINT'S EAST BOUNDARY.  IT IS REPORTED THAT HYDROGEN |
| WILL BE VENTED FOR SEVERAL HOURS.  WIND DIRECTION IS FROM FITZPATRICK SITE   |
| TOWARDS NINE MILE POINT.                                                     |
