DEFECTS ON FOUR MOTOR OPERATED VALVES SUPPLIED BY THE HENRY PRATT
COMPANY OF AURORA, ILLINOIS.
                                                                 
DURING SURVEILLANCE TESTING OF THE RECIRCULATION SPRAY PUMP
SYSTEM, A PROBLEM WITH ONE OF THE PUMP SUCTION VALVES WAS
DISCOVERED. INVESTIGATION REVEALED THAT THE VALVE DISC HAD BECOME
DISENGAGED FROM THE MOTOR OPERATOR.
                                                                 
THE PROBLEM WAS DETERMINED TO BE A RESULT OF AN EXCESSIVE
DIAMETER HOLE ON THE VALVES ADAPTER PLATE WHICH ALLOWED THE
SPLINE ADAPTER ON THE MOTOR-OPERATOR TO MOVE OUT OF POSITION. THE
DEFICIENCY WAS FOUND ON ALL FOUR RECIRCULATION SPRAY PUMP SUCTION
VALVES. THE VALVES ARE 12" PRATT BUTTERFLY VALVES WITH LIMITORQUE
HBC OPERATORS.
                                                                 
AN INSPECTION OF THE ADAPTER PLATES OF ALL SIMILAR HENRY PRATT
VALVES ON BOTH UNITS 1 AND 2 DETERMINED THAT THE FOUR
RECIRCULATION SPRAY PUMPS SUCTION VALVES WERE THE ONLY VALVES
AFFECTED BY THIS DEFECT. THE LICENSEE STATES THAT NO WORK
ACTIVITY OR MODIFICATIONS WERE EVER CONDUCTED ON THE VALVE
ADAPTER PLATES AND THAT THEY CONSIDER THAT THE DEFECTS ORIGINATED
WITH THE MANUFACTURER.
                                                                 
A TEMPORARY MODIFICATION HAS BEEN MADE TO THE FOUR SUCTION VALVES
WHICH WILL PROPERLY POSITION AND RETAIN THE SPLINE ADAPTER.
INSTALLATION OF THE CORRECT REPLACEMENT HARDWARE WILL BE
SCHEDULED FOR THE NEXT REFUELING OUTAGE.
