A WYLE LAB REPORT RESULTED IN THE CALCULATION OF A
NON-CONSERVATIVE SERVICE LIFE, AND CONSEQUENTLY A
NON-CONSERVATIVE CHANGE-OUT FREQUENCY WAS ESTABLISHED FOR VARIOUS
AGASTAT GP RELAYS.
                                                                 
THE WYLE LAB REPORT STATED THAT VARIOUS AGASTAT GP SERIES RELAYS
HAD A SERVICE LIFE OF 26.3 YRS WHICH NIAGARA MOHAWK HAD
IMPLEMENTED IN THEIR PROCEDURES. THE REPORT USED AN ACTIVATION
ENERGY, FOR CALCULATING THE LIFE OF THE RELAY BOBBIN MATERIAL,
WHICH WAS HIGHER THAN WHAT THEY SHOULD HAVE USED FOR THAT
MATERIAL. IF THEY HAD USED THE LOWER MORE CONSERVATIVE VALUE, THE
SERVICE LIFE OF THE RELAYS WOULD HAVE BEEN LESS. WITH THIS
CONDITION EXISTING, THERE WAS A POTENTIAL FOR THE CORE SPRAY
SYSTEM BECOMING INOPERABLE WITH THE FAILURE OF THESE RELAYS.
IMMEDIATE CORRECTIVE ACTION WAS TO REPLACE THE SUBJECT AGASTAT GP
