PART 21 REPORT ON THE POTENTIAL FAILURE OF A MAIN STEAM SAFETY   
VALVE                                                            
                                                                 
A DRESSER MAIN STEAM SAFETY VALVE FROM ENTERGY OPERATIONS, INC., 
ARKANSAS NUCLEAR ONE (ANO) UNIT 1, WAS REFURBISHED AT WYLE       
LABORATORIES, HUNTSVILLE, ALABAMA, IN FEBRUARY 1995.  THIS WAS A 
TYPE 3707R SAFETY VALVE WITH SERIAL NUMBER BM-02978.  DURING THE 
REFURBISHMENT, A DRESSER SERVICE ENGINEER FOUND SEVERAL          
NONCONFORMANCES.  THE NONCONFORMANCES DISCOVERED DURING THE      
REFURBISHMENT INCLUDED THE USE OF FOREIGN REPLACEMENT PARTS, THE 
USE OF IMPROPER TOOLS AND WORN PARTS.  THESE NONCONFORMANCES MAY 
HAVE CAUSED THE VALVE TO NOT FUNCTION AS DESIGNED.
