Part 21 2005-0035-00 - Governor Drive Coupling Element

Engine Systems Inc. (ESI) has concluded an investigation into a condition reported by the Shearon Harris Nuclear Power Plant with regard to the governor drive coupling element, P/N AK-007-001. ESI concluded that it has supplied defective coupling elements to several utilities that are fabricated from an incorrect material.

The results of ESI's internal investigation revealed that several utilities have been supplied coupling elements made from isoprene. The isoprene material is not suitable for the high temperature, oil rich environment of the engine gear case and is susceptible to failure after prolonged exposure.

Affected sites are Grand Gulf, River Bend, Perry, Vogtle, Shearon Harris, and Comanche Peak.

Part 21 2005-0030-01 - Struthers-Dunn 219 Series Relays Manufactured in China

On August 2, 2005, Duke Energy Corporation notified NRC (Event Number 41888) that it had purchased Struthers-Dunn 219 Series relays with potential manufacturing defects from Newark Electronics, a distributor for Struthers-Dunn relays. These relays were purchased as commercial grade and were subsequently dedicated for use in nuclear safety related applications. The affected relays have been identified by the supplier as having a manufacturing defect in the relay coil. Some of the coils did not have their coil cores annealed due to omitting the annealing step in the manufacturing process. Because the defective coils draw more current, they may subsequently fail after an extended time in the energized state. The safety hazard which is created or could be created by the defect depends upon the application in which the affected relays are utilized.

These relays were manufactured in China with AC coils that have a date code of the 24th week of 2004 or earlier (date codes of 0424B or earlier). The letter "B"occurs at the end of the affected date codes which identifies the manufacturing location where the defective relays were manufactured.

Catawba Nuclear Station previously utilized five of the affected relays in applications associated with the safety related Nuclear Service Water System, specifically, in the control circuitry for various isolation valves in the system. All of the affected relays that were installed in the plant have since been replaced with relays utilizing coils that have been properly annealed. In addition, three of the affected relays were located in inventory and these relays have since been removed from inventory. McGuire Nuclear Station does not have any of the affected relays installed in nuclear safety related applications. Oconee Nuclear Station has not installed any of the affected relays.

The NRC issued Morning Report H-2005-0021 on August 17, 2005 when this issue was first reported by Catawba Nuclear Plant. NRC continues to evaluate this issue.
