CONCURRENT REPORT FOR OFFSITE NOTIFICATION
The following is a summary of the information provided by the licensee via phone and email:
Centrus American Centrifuge Operating (ACO) received notification from a Fluor-BWXT Portsmouth (FBP) environmental technician supervisor that the chlorine level at outfall '013/X-2230N' was at 0.159 mg/L. This exceeds the National Pollutant Discharge Elimination System (NPDES) permit limit on total chlorine at outfall '013' of 0.05 mg/L. Centrus ACO notified the Ohio Environmental Protection Agency at 1539 EDT.
Centrus ACO asked FBP to resample on the morning of 8/19/2025 to further confirm the exceedance. Centrus ACO is currently investigating to determine the cause of the exceedance. This report is in accordance with condition notification #12803.
The NRC regional office was notified.

AGREEMENT STATE REPORT - DAMAGED RADIOGRAPHY DRIVE ASSEMBLY
The following is a summary of information provided by the Georgia Radioactive Materials Program via email:
The licensee reported that a source could not be returned to the locked shielded position following an exposure. A licensee radiation safety officer (RSO) reported to the scene with a source retrieval kit and attempted to retract the source. Initial attempts at straightening the guide tube were unsuccessful. Subsequently, it was noticed that a portion of the drive assembly conduit (return side) was melted due to being too close to a drop light. The drive assembly conduit (projection side) was disconnected from the crank handle and the source was successfully retracted into the locked shielded position by pulling the drive cable by hand. The exposure device was surveyed and secured.
While the source was exposed, a 2 mr/hr perimeter was established with continuous surveillance. The maximum dose received by a worker was to the RSO, who received 68 mrem, as indicated by a pocket dosimeter. The drive assembly will be removed from service until repairs can be made by the manufacturer.

AGREEMENT STATE REPORT - LOSS OF LICENSED MATERIAL
The following information was provided by the Illinois Emergency Management Agency (the Agency) via email:
"As a result of an ongoing Agency investigation into unresponsive general licensees, the Agency became aware today [August 19, 2025], that a generally licensed X-ray fluorescence device, containing 30 millicuries of Am-241, was improperly disposed of. Reportedly, on or about June 3, 2025, Midland Manufacturing Company mailed a Niton XLp-818Q (serial number 8270) to a Veolia electronics recycling facility in Port Washington, WI. Both [the Agency] and Wisconsin program staff are attempting to contact the facility to determine if the device is still available for recovery. The quantity of radioactive material involved, while unlikely to be dangerous to the public, is immediately reportable to the Agency and the U.S. Nuclear Regulatory Commission. The investigation remains ongoing."

AGREEMENT STATE REPORT - LOST TREATMENT VIAL
The following information was provided by the Nevada Division of Public and Behavioral Health via phone and email:
"On July 8th, a Xofigo package [containing liquid Ra-223 dichloride solution of an unknown total activity] was checked in, and subsequent procedures were carried out for dose verification and disposal. However, the dose was later found to be missing, prompting an investigation.
"Upon receipt, the surface radiation level of the package was measured at 1.8 mR/hr. Wipe tests performed on both the inside and outside of the package registered 122 dpm. [There is no contamination concern at this time.]
"The dose was then checked into inventory as per protocol. The radioactive warning symbols on the package were defaced and physically removed. The package box was placed around the corner for the cleaning staff to dispose of.
"The following day, when attempting to retrieve the dose from the storage cabinet, it was discovered that the dose was missing. It was realized that during the initial wipe test, the dose had not been removed from the box before disposal. Efforts to locate the missing dose included checking outside trash bins, but these had already been emptied that morning. [The licensee] contacted maintenance personnel who liaised with the cleaning company and waste management services. Waste management confirmed that there is no method available to locate the missing dose and that it is likely disposed of in the main trash area.
"The Xofigo packaging weighs nearly the same whether it contains the dose or not, due to the plastic pig inside. To prevent recurrence, a new process has been implemented: the Styrofoam lid will be removed and discarded immediately after wiping to confirm the box is empty. Additionally, the entire box will now be discarded only after the dose has been administered to the patient."
