The package was received on 4/8/18 around 0700 EDT. Wipe tests performed on the external surface of the package indicated a removable contamination level of 8,427 dpm/300 cm2. Radiation levels of the package measured 0.02 millirem at the surface. The package contained Technetium-99m and was shipped by Cardinal Radiopharmacy of Jenison, MI. No damage to the package was observed. The hospital employee receiving the package immediately notified the shipper about the contaminated package. Surveys were performed in the package receipt area to ensure that contamination was not spread beyond the area. No additional contamination was found in the package receipt area. | |
