| This notification retracts an earlier report made at 1916 on 12/21/98 per    |
| 10CFR50.72(b)(2)(iii)(B) and (D), due to observed flow oscillations during   |
| HPCI System testing.  On 12/23/98, the licensee determined that the flow     |
| oscillation did not affect the operability of the HPCI System and this event |
| notification should have been retracted at that time.                        |
|                                                                              |
| During HPCI System testing following a refueling outage, the HPCI System was |
| declared inoperable due to observed flow spiking indication on start up of   |
| the system.  Upon further investigation, the observed oscillations were      |
| attributed to spiking on system startup due to either air in the flow        |
| transmitter or electronic noise providing a false momentary indication.  In  |
| either case, it was demonstrated that the HPCI System was not affected by    |
| the momentary spiking indication and the system was able to perform its      |
| function and was determined to be operable.  Therefore, this event           |
| notification is being retracted.                                             |

| THREE SPRINKLER SYSTEMS DECLARED INOPERABLE DUE TO CORRODED HEADS (24-hour   |
| report)                                                                      |
|                                                                              |
| The following text is a portion of a facsimile received from Paducah:        |
|                                                                              |
| "On 06/02/99 at 1630 CDT, the Plant Shift Superintendent (PSS) was notified  |
| that numerous sprinkler heads were corroded, affecting 16 sprinkler systems  |
| in C-337 and one system in C-333, such that the ability of the sprinklers to |
| flow sufficient water was called into question.  Subsequently, these         |
| sprinkler systems were declared inoperable, and TSR-required actions         |
| establishing roving fire patrols were initiated.  This deficiency was        |
| detected during scheduled system inspections conducted by Fire Protection    |
| personnel.  Currently, functionality of the sprinkler heads has not been     |
| fully evaluated by Fire Protection personnel, and the remaining cascade      |
| buildings are currently being inspected, and if necessary, this report will  |
| be updated to identify any additional areas.                                 |

| condition at 1350 CDT on 06/18/99.  The area of the fire patrol for system   |
| C-15 was expanded to include the two heads identified as corroded.  The one  |
| head on system B-8 was in the area already being patrolled.  The PSS         |
| determined that an update to this report was required."                      |

| time frame required by TSR.  The PSS determined that an update to this       |

| fire patrols for the affected areas were initiated in the time frame         |
| required by the TSR. The PSS determined that an update to this report was    |
| required. The NRC resident inspector has been informed of this update.       |

| inoperable at 1227 CDT and System B-8 inoperable at 1406 CDT.   An LCO       |
| required fire patrol for the affected areas was initiated in the time frame  |
| required by the TSR..  The PSS determined that an update to this report was  |
| required.  The NRC Resident Inspector has been informed of this update.  The |
| NRC Operations Officer notified R3DO Bruce Jorgensen.                        |

| EMERGENCY SERVICE WATER SYSTEM DEGRADATION (ONLY ONE OF FOUR PUMPS           |
| OPERABLE)                                                                    |

| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "On 08/26/99, Susquehanna Unit 1 and Unit 2 were operating at 100% power     |
| with the 'B' loop of emergency service water [(ESW)] out of service for      |
| scheduled maintenance.  During testing on the ESW system, with all ESW pumps |
| in service, it was identified that the 'C' and 'D' ESW pumps' discharge      |
| check valves were closed.  The ESW flow surveillance was performed, and the  |
| 'C' and 'D' ESW pumps failed to achieve the required flow and were declared  |
| inoperable.  Concurrently, the 'B' loop of ESW was returned to service.      |
| During the time the 'B' ESW loop was inoperable, the 'A' ESW pump was the    |
| only one operable ESW pump.  This constitutes a serious degradation of the   |
| plant in that it is a condition which is outside of design basis and,        |
| therefore, reportable under 10 CFR 50.72(b)(1)(ii) requiring a 1-hour ENS    |
| notification."                                                               |

| The licensee notified the NRC resident inspector.                            |

| The licensee is Retracting the Retraction they called in on 09/03/99 at      |

| "On 09/22/99 at 2230, an Engineering evaluation concluded that the           |
| operability determination associated with the above stated event was not     |
| adequate, specifically relating to the potential for pump interaction to     |
| create low flow conditions. "                                                |
|                                                                              |
| "The Engineering evaluation was being performed in response to an anomaly    |
| identified on 09/20/99 during the run of 'D' Diesel Generator.  During the   |
| preparation of the operability statement for the 09/20/99 anomaly,           |
| engineering personnel realized that incorrect assumptions were used in the   |
| previous operability statement.  The 'C' and 'D' ESW pumps were declared     |
| inoperable at 2230 on 09/22/99.  Evaluation continues to determine if other  |
| combinations of equipment out of service times result in additional          |
| reportable conditions."  R1DO (Bob Summers) notified.                        |

| The NRC Resident Inspector will be notified of the Retraction to the         |
| Retraction of this event by the licensee.                                    |

| Based on additional evaluations by Licensee Engineering, Operations and      |
| Licensing spanning the time frame of 06/03/99 to 09/28/99, the licensee      |
| concluded that the plant had at least one Emergency Service Water Pump per   |
| loop capable of performing its specified safety function at all times.  As   |
| such, the plant was not in a condition outside of its design basis, was not  |
| in an unanalyzed condition, and there was no loss of safety function.  The   |

| NONCOMPLIANCE WAS DISCOVERED WHILE REVIEWING OBSERVATIONS FOUND DURING A     |
| WALKDOWN OF IMPLEMENTED NCSAs.                                               |

| AT COLUMN C-53 ON THE CELL FLOOR OF X-330, A DRIP PAN CONNECTED TO A PAIR OF |
| LUBE OIL STRAINERS DID NOT MEET THE REQUIREMENTS OF NCSA-PLANT054 FOR LUBE   |
| OIL SYSTEMS. NCSA-PLANT054 REQUIRES ALL LUBE OIL LEAK COLLECTION CONTAINERS  |
| IN AREAS WHERE SIGNIFICANT UF6 RELEASES ARE CONSIDERED CREDIBLE, MEET        |
| CERTAIN SIZE RESTRICTIONS. IN X-330, THE MAXIMUM ALLOWED DEPTH OF A LUBE OIL |
| DRIP PAN ON THE CELL FLOOR IS 3.6 INCHES. THE DRIP PAN FOUND WAS 5.76 INCHES |
| DEEP. THIS VIOLATES THE CONTAINER SIZE REQUIREMENT OF THE NCSA. THE DRIP PAN |
| CONTAINED APPROXIMATELY 25 QUARTS OF OIL AT TIME OF DISCOVERY. THERE WAS NO  |
| EVIDENCE OF ANY UF6 RELEASES IN THE AREA.                                    |

| On 10/21/ 1999 at 1500 hrs the Plant Shift Superintendent(PSS) was notified  |
| that a Nuclear Criticality Safety Approval (NCSA) requirement was not being  |
| performed in the X-710 ES&H Analytical Labs. NCS Engineering, while          |
| performing an NCS Self Assessment, noticed an operation where samples were   |
| brought to a staging area "INSIDE" a room when they were logged in and       |
| verified. (NCSA-0710_025.AOO)                                                |
|                                                                              |
| Requirement #1 of NCSA-071_25.AOO states in part; Samples in any room        |
| enriched to 1% or greater U-235, will be limited to one group of 30g U-235   |
| per room. This will be ensured by maintaining a room inventory log of U-235  |
| mass in labeled containers. Each log entry will consist of the U-235 mass    |
| value plus analytical uncertainty and will be:                               |
|                                                                              |
| a. made by a knowledgeable person "PRIOR" to bringing the samples into the   |
| room, and                                                                    |
| b. verified by a second knowledgeable person "PRIOR" to bringing the samples |
| into the room.                                                               |
|                                                                              |
| At the direction of the Plant Shift Superintendent the requirements for an   |
| NCS anomalous condition were initiated and the area baundaried off.          |
|                                                                              |
| The safety significance of the event is very low. The U-235 mass in material |
| is being maintained less than 30 grams as required. This is more than a      |
| factor of 10 less than the always safe mass of 350 grams U-235. The samples  |
| were brought to a staging area inside the room where they were logged in and |
| verified. After logging and verifying the samples were moved to the          |
| operational area of the room, where they could interact with other materials |
| in the room.                                                                 |
|                                                                              |
| Samples in any room enriched to 1% or greater U-235, with a concentration    |
| greater or equal to 500 ppm U-235, shall be limited to one group of 30 g     |
| U-235 per room. This shall be ensured by maintaining a room inventory log of |
| U-235 mass in labeled containers. Each log entry shall consist of the U-235  |
| mass value plus analytical uncertainty and shall be:                         |

| Mass is the only parameter controlled                                        |
|                                                                              |
| The actual amount of material In the room is less than 30 grams U-235, at an |
| enrichment of less than or equal to 100%. This value is obtained from sample |
| data that was logged after the material was brought Into the room. This is   |
| approximately 4% of the worst case critical mass.                            |
|                                                                              |
| Verbatim compliance with both mass controls was lost because the material    |
| was supposed to be checked and verified prior to bringing the material into  |
| the room. The samples were brought to a staging area inside the room where   |
| they were logged in and verified. After logging and verifying the samples    |
| were moved to the operational area of the room, where they could interact    |
| with other materials in the room.                                            |
|                                                                              |
| THERE WAS NO LOSS OF HAZARDOUS/RADIOACTIVE MATERIAL OR                       |
| RADIOACTIVE/RADIOLOGICAL CONTAMINATION EXPOSURE AS A RESULT OF THIS EVENT.   |
|                                                                              |
| The Resident Inspector was notified and the DOE Representative will be       |
| notified.                                                                    |
