Subsequently, these | | sprinkler systems were declared inoperable, and TSR-required actions | | establishing roving fire patrols were initiated. This deficiency was | | detected during scheduled system inspections conducted by Fire Protection | | personnel. The one | | head on system B-8 was in the area already being patrolled. Both systems were immediately declared inoperable and LCO | | fire patrol actions were implemented. This condition was discovered as a result of an | | engineering review of individual sprinkler heads that had previously been | | identified as exhibiting corrosion around the valve seat. The engineering | | review was conducted following the discovery that similar corrosion problems | | experienced at the Paducah Gaseous Diffusion Plant resulted in sprinkler | | heads being unable to actuate at normal pressure. An initial screening of | | PORTS sprinkler heads known to exhibit corrosion was conducted, and systems | | where the potential for two or more adjacent heads to be corroded were | | inspected. This identified 11 systems in the X-333 and 2 systems in the | | X-326 that did not meet the sprinkler system operability requirement that no | | two adjacent sprinkler heads be inoperable." | | | | "The PSS declared the affected sprinkler systems inoperable, and TSR | | required actions were completed. Further inspections of sprinkler systems | | are planned. This report will be updated if additional sprinkler systems | | are determined to be inoperable." | | | | "There was no loss of hazardous/radioactive material or | | radioactive/radiological contamination exposure as a result of this | | event." | | | | Portsmouth personnel notified the NRC resident inspector as well as the | | onsite Department of Energy representative. | | | | *** UPDATE ON 8/19/99 AT 2327 FROM HALCOMB TO POERTNER *** | | | | During continuing walkdowns, HPFW system #398 was discovered with two or | | more (3) adjacent sprinkler heads with mineral deposits. During testing on the ESW system, with all ESW pumps | | in service, it was identified that the 'C' and 'D' ESW pumps' discharge | | check valves were closed. The ESW flow surveillance was performed, and the | | 'C' and 'D' ESW pumps failed to achieve the required flow and were declared | | inoperable. Concurrently, the 'B' loop of ESW was returned to service. | | During the time the 'B' ESW loop was inoperable, the 'A' ESW pump was the | | only one operable ESW pump. Please see the attached report for derails | | involving this event. | | | | "This letter is sent concerning the American Ecology Recycle Center d/b/a | | U.S. Ecology Nuclear Materials Management Center (NMMC) Radioactive Material | | License R-01037-B04. The NMMC is providing written documentation of the | | reportable event that was reported to your office via telecom on August 6, | | 1999. The event involved receipt of a radioactive material shipment with | | external radiation levels in excess of those reportable under State | | Regulations for Protection Against Radiation (SRPAR) Section | | 1200-2-5-.16(2)(d). | | | | "Investigation: | | | | "The subject event involved the U.S. Ecology Brokerage division truck and | | driver on-loading a shipment consisting of a source drum at CTL Engineering, | | Columbus, OH. Receipt surveys performed at the NMMC denoted an on-contact | | reading of 450 mR per hour (i.e., greater than the reportable 200 mR per | | hour) at a single point on the bottom of the truck. The location of the | | reportable radiation level was not such that it would provide a credible | | exposure opportunity for a member of the public. | | | | "After on-loading the subject source drum, the truck made eight more stops | | throughout the mid-west (i.e., OH, MI, IL, IN) over a period of 7 days, with | | each stop ranging in duration between approximately � hour and 4 hours per | | stop. Interviews with the driver indicate that each stop was in an | | industrial location with no notable personnel activity (i.e., occupancy) | | around the truck. The driver also noted that he makes a conscious effort to | | park away from others when making nightly stops. Therefore, he did not note | | any personnel activity around his truck that could have led to a credible | | exposure opportunity for a member of the public. | | | | "Safety Significance: | | | | "There is no plausible scenario in which a member of the public was | | over-exposed due to this event, The driver, who was the maximally exposed | | individual, was issued DOSIMETRY of record at the time of the event. His | | record dose will be evaluated upon the subject TLD being processed. | | | | "Cause: | | | | "The most plausible cause for the event was that the driver made a human | | performance error while reading his survey meter. In June 1999, the reliability engineer with the 'systems | | group' department of our company sent me a memo that suggested we might have | | accepted an unacceptable part for the 30-4 connector, which is a regulated | | component. He had connected an AMP BNC connector to a cable to determine | | its equivalency to the other qualified part from Amphenol p/n 31-2. He | | found that when the wires were combed out as directed on the instructions | | that came with the AMP connector, it did not appear to perform correctly | | with respect to cable retention, and [he] asked if someone in QA could | | verify his preliminary testing. | | | | "Subsequently, our Quality Manager examined the connector and obtained | | instructions from the manufacturer that said the wires should be left | | braided, not combed out, and that if you left it braided, a stronger grip | | was achieved. The reliability engineer tried the braided connection and did | | find that the retention was improved. However, in investigating the matter | | further with the manufacturer, it was found that the manufacturer did have | | conflicting instructions but said the drawing specifying a braided | | connection should be considered the official one since it was a controlled | | document. It was also found that the part from AMP was not the originally | | qualified part number - 221552-1. AMP had obsoleted that part in 1992 and | | substituted the 221265-0 in its place. The new part, however, did not carry | | a military specification for shock (MIL-Std. 202) and was not military type | | UG-88 like the previous one - conditions which provided assurance that it | | met certain criteria needed for Seismic Class 1 rating. This information | | was presented at the company's Quality Review Meeting on August 12, 1999. A | | technical evaluation was conducted and reported in an abstract for the 30-4 | | BNC Connector by the reliability engineer on August 26, 1999. | | | | "The connectors for part 30-4 that were in stock were inspected, and it was | | found that all of them were this incorrect AMP part number. The connectors | | were subsequently put on hold by the Quality Manager, and correct connectors | | have been ordered from Amphenol. | | | | "One hundred pieces of the incorrect part (AMP 1-221265) were received at | | Victoreen (now a part of Inovision) in January 1997. A search has been | | conducted to identify those parts, assemblies, and kits which list part 30-4 | | as part of their bill of material. From those, a review will be done to | | identify those that have been produced since the receipt of the incorrect | | part and, therefore, have the potential for being incorrect. The customers | | who have received the identified products will be sent a notification within | | 30 days. If acceptable replacement parts can be obtained with that time | | frame, they will be sent with the notification. | | | | "A notification was sent to the Plant Manager/Senior Executive and to the | | Chief Operating Officer of Inovision on August 26, 1999.
