| At 1500 EDT on 04/21/99, Ken Traegde of the Massachusetts Department of      |
| Public Health Radiation Control Program (RCP) located in Boston,             |
| Massachusetts, reported that on 04/16/99, AEA Technology QSA, Inc., located  |
| in Burlington, Massachusetts, shipped a Model-855 source changer loaded with |
| eight Iridium-192 sources (totaling 866 curies) via Federal Express to       |
| Vicont S.A. De C.V. located in Toluca, Mexico.  The undamaged package had a  |
| transport index of 1.4 (normal readings of 1.4 mR/hour at 1 meter and 55     |
| mR/hour at the surface) at the time of shipment.                             |
|                                                                              |
| The shipment was scheduled to arrive in Mexico on 04/17/99.  As of 1400 EDT  |
| on 04/21/99, the recipient (Vicont S.A. De C.V.) had received the shipping   |
| papers but not the container.  The shipper and Federal Express have launched |
| a full search for the package.  The Federal Express Hazardous Material       |
| Investigation Team has conducted a physical search of Express Depots in      |
| Boston, Memphis, Los Angeles, and Mexico.                                    |

| The license numbers are:  SU-1485, 12-8361, 52-0243, 54-0237, and 58-0242.   |

| A conference call was conducted at 1230 on 04/23/99 to update the status of  |
| this event.  The following NRC offices participated (among others):  IRO     |

| and Woodruff), Region 4 (Andrews), OPA (Hayden), OIP (Lieberman), OSP        |
| (Lohaus), and EDO (Raddatz).  In addition, the following agencies            |
| participated:  Department of Transportation (Rick Boyle), Massachusetts      |
| Department of Public Health RCP (Ken Traegde), Federal Express (Scott Mugno, |

| During this conference call, representatives of Federal Express reported     |
| that they are confident that the shipment of eight Iridium-192 sources left  |
| Boston.  They issued an 'all-points' search order, faxed a description to    |
| all 'logical' locations, and conducted repeat physical searches in Memphis,  |
| Boston, and Mexico, all without locating the shipment to date.  (The Los     |
| Angeles depot was not searched as had been reported on 04/21/99.)            |
| Massachusetts and Tennessee health authorities conducted radiological        |
| surveys in Boston and Memphis on 04/22/99 with negative results.             |
|                                                                              |
| Rick Boyle of the Department of Transportation notified the competent        |
| authority in Mexico, and Federal Express requested a detailed search of the  |
| Customs facilities in Mexico City.                                           |
|                                                                              |
| The next conference call is scheduled for 1230 EDT on Wednesday, 04/28/99.   |

| At 0619 EDT on 04/26/99, Federal Express confirmed that the serial number on |
| a Model-855, Type-B package found at the Stansted Airport (located outside   |
| of London, England) matched the serial number for the missing package        |
| containing the eight Iridium-192 sources.  The Massachusetts Department of   |
| Public Health RCP was informed of this discovery by AEA Technology QSA,      |
| Inc., at 0933 EDT on 04/26/99.  The package was reported to be intact and    |
| undisturbed.  The carrier (Federal Express) contacted the shipper (AEA       |
| Technology QSA, Inc.), and the shipper requested that the package be         |
| forwarded to its intended destination.  Accordingly, the package will be     |
| shipped from Stansted Airport to Toluca, Mexico, via Memphis, Tennessee.     |
| Federal Express informed the Massachusetts Department of Public Health RCP   |
| that the package is expected to arrive in Memphis, Tennessee, by tomorrow    |
| morning (04/27/99).                                                          |

| The Massachusetts Department of Public Health RCP notified NRC Region 1      |
| (Duncan White).  The NRC Operations Officer notified the R1DO (Anderson);    |

| POSSIBLE OVERLOAD OF ESF TRANSFORMER IN AN ABNORMAL OPERATING PROCEDURE      |

|                                                                              |
| While investigating a previous Licensee Event Report concerned with backfeed |
| the following was discovered:                                                |
| "During a scenario involving a Loss of Off-Site Power resulting from a loss  |
| of the BEST and Startup transformers, the Crystal River Unit 3 abnormal      |
| operating procedures could have caused an overload of the safety-related     |
| Engineered Safeguards 4160V to 480V transformer.  Assuming a single failure  |
| of the 'B' side diesel generator, power could have been unavailable to both  |
| trains of 480V Engineered safeguards loads.  This condition is considered    |
| reportable per 10CFR50.72(b)(l)(ii)(B) and 10CFR50.73(a)(2)(ii)(B) as a      |
| condition outside of the design basis of the plant.  Loads have been         |
| identified which can be administratively maintained off during this scenario |
| to avoid the overload condition, and therefore, the transformer is           |
| operable."                                                                   |
|                                                                              |
| This condition is under investigation and further actions will be            |
| forthcoming.                                                                 |
|                                                                              |
| The licensee intends to notify the NRC Resident Inspector.                   |
|                                                                              |
| **** UPDATE ON 05/07/99 AT 2311 BY RAWLS TO GOULD ****  EVENT RETRACTION     |

| THE LICENSEE AND ABB POWER T&D CO, INC HAVE EVALUATED POSSIBLE OVERLOAD      |
| CONDITIONS OF THE "A" 4160/480V TRANSFORMER DURING A LOSS OF OFFSITE POWER   |
| WITH THE LOSS OF THE "B" BATTERY SCENARIO.  THE RESULTS OF THE EVALUATIONS   |
| SHOW THAT THE CLASS 1E TRANSFORMER AND COMPONENTS WILL PERFORM THEIR         |
| DESIGNED FUNCTION FOR LONGER THAN 30 DAYS WITHOUT FAILURE.  THE TRANSFORMER  |
| WILL SURVIVE LONGER THAN ONE YEAR UNDER THE IDENTIFIED LOAD CONDITIONS BASED |
| ON A REVIEW OF ANSI/IEEE C57.96.  IN ADDITION, THE NON-CLASS 1E CROSSTIES    |
| CIRCUITS TO THE RX AUX BUS 3A/3B ARE LICENSING BASIS REQUIREMENTS AND ARE    |
| NOT DESIGN BASIS REQUIREMENTS.  THE LICENSEE IS THEREFORE RETRACTING THIS    |
| EVENT.                                                                       |

| THIS OCCURRED WHEN AN ELECTRICIAN LIFTED A LEAD WHILE REPLACING RELAY        |
| "2-CLS-RLY-1B1" IN THE "B" TRAIN HIGH CLS SYSTEM.  ONE CONTAINMENT TRIP      |
| VALVE FOR THE CONTAINMENT PARTICULATE AND GAS RADIATION MONITORS             |
| REPOSITIONED (CLOSED), AND THE PARTICULATE AND GAS RADIATION MONITOR PUMP    |
| DE-ENERGIZED.   A SAFETY INJECTION SIGNAL WAS NOT RECEIVED AS IT WAS         |
| PROPERLY DEFEATED IN ACCORDANCE WITH STATION PROCEDURES.  THE ELECTRICIANS   |
| VERIFIED THAT THE HIGH CLS SYSTEM WAS DE-ENERGIZED AS THEY WOULD EXPECT      |
| PRIOR TO PERFORMING WORK TO PREVENT A SPURIOUS ACTUATION.  THE LICENSEE IS   |
| INVESTIGATING THE CAUSE AND THE REQUIRED CORRECTIVE ACTIONS.  THE HIGH CLS   |
| SIGNAL WAS RESET SINCE THE 2/4 CONTAINMENT PRESSURE < 14.2 PSIA MATRIX WAS   |
| MET DUE TO THE ACCURACY OF THE GAUGES.  THE TRIP VALVE WAS PLACED IN THE     |
| PRE-EVENT CONDITION (OPEN), AND THE RADIATION MONITOR PUMP WAS RE-ENERGIZED  |
| AND RESTARTED.  CORE OFF LOAD WAS NOT IN PROGRESS AT THE TIME OF THIS        |
| EVENT.                                                                       |

| The licensee is retracting this event notification.                          |
|                                                                              |
| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "At 1113 on April 26, 1999, a 4-hour Non-Emergency Notification was made in  |
| accordance with 10CFR50.72(b)(2)(ii) due to an ESF actuation.  An            |
| inadvertent Unit 2 High Consequence Limiting Safeguards (CLS) signal for     |
| Train 'B' was caused by maintenance activities when replacing a High CLS     |
| relay (#2-CLS-RLY-1B1) with Unit 2 in Refueling Shutdown.  The High CLS      |
| signal only initiated the closure of containment trip valve #2-RM-TV-200B,   |
| which caused the sample pump, #2-RM-P-200, for the containment particulate   |
| and gaseous radiation monitors to de-energize."                              |
|                                                                              |
| "NUREG-1022, Event Reporting Guidelines 10CFR50.72 and 50.73, states that    |
| 'The Commission is interested both in events where an ESF was needed to      |
| mitigate the consequences (whether or not the equipment performed properly)  |
| and events where an ESF actuated unnecessarily.... Usually, the staff would  |
| not consider this to include single component actuations because single      |
| components of complex systems, by themselves, usually do not mitigate the    |
| consequences of significant events.'"                                        |
|                                                                              |
| "Based on further review of this event and guidance provided in NUREG-1022,  |
| it was determined that this event was not reportable as the High CLS signal  |
| was invalid and resulted in the actuation of only one component which did    |
| not mitigate the consequences of the event."                                 |
|                                                                              |
| "This notification is made to retract the report made on April 26, 1999,     |
| based on the above discussion."                                              |

| DISCOVERY OF A MISSING CHEMICAL AGENT MONITOR CONTAINING 15 mCi OF NICKEL-63 |
| FROM SHAW AIR FORCE BASE LOCATED IN SUMPTOR, SOUTH CAROLINA                  |

| At 0938 on 05/07/99, Shaw Air Force Base (AFB) located in Sumptor, South     |
| Carolina, notified the U.S. Air Force Radioisotope Committee located at      |
| Bolling AFB located in Washington D.C. of a missing chemical agent monitor   |
| (CAM) containing a 15-mCi nickel-63 source.                                  |
|                                                                              |
| On 04/27/99, the base radiation safety officer (RSO) at Shaw AFB conducted   |
| an inventory of CAMs located in building #1981.  The correct number of CAMs  |
| were there, but one had the wrong serial number.  Examination of previous    |
| inventories shows one CAM with a different serial number.  The building had  |
| been searched with no success.  The 20CES/CEX RSO has no explanation for the |
| different serial number on one CAM or where the correct CAM may be located.  |

| It is suspected that the CAM may have been exchanged with another Air Force  |
| Unit or may have been sent off for calibration.  At this time, the U.S. Air  |
| Force Radioisotope Committee believes that the CAM is still in Air Force     |
| possession.  Past shipping records will be researched, and calibration labs  |
| that serve Shaw AFB will be notified.  The completion time for these actions |
| is currently estimated to be 06/07/99.                                       |

| AUTOMATIC REACTOR SCRAM DUE TO LOSS OF EHC PRESSURE                          |
|                                                                              |
| An automatic reactor scram occurred due to high reactor vessel pressure      |
| (1080 psig) after the main turbine stop and control valves drifted closed.   |
| All control rods inserted following the scram, and all systems functioned as |
| expected.  Neither the HPCI nor RCIC systems started, as reactor vessel      |
| water level did not decrease to the automatic actuation setpoint.  The       |
| licensee reported that an operator inadvertently isolated the main turbine   |
| electrohydraulic control (EHC) system oil supply while tagging out the       |
| standby EHC oil pump, causing the turbine stop and control valves to drift   |
| closed.                                                                      |

| The NRC Resident Inspector has been informed of this notification by the     |
| licensee.                                                                    |
