Manual License Verification Procedure

The purpose of this procedure is to guide licensees transferring Category 1 and/or Category 2 quantities of radioactive materials to verify licenses outside of the License Verification System (LVS), and for Regulatory Agencies to respond to such requests.

Steps for Licensees:

  1. Contact the LVS Help Desk by phone at 1-877-671-6787 or by email at LVSHelp.Resource@nrc.gov.

  2. Be prepared to provide the following information to the LVS Help Desk representative:

  3. Transferring Licensee information:

    1. License-issuing Agency
    2. Licensee name
    3. License number
    4. Contact name and title
    5. Contact phone
    6. Contact email
    7. Contact fax number

    Receiving licensee information:

    1. License-issuing Agency
    2. Licensee name
    3. License number
    4. Amendment number or license issue date
    5. Authorized storage location address
    6. Material(s) being requested
    7. Chemical/Physical form of the material(s) being requested
    8. Quantity/Activity being requested
  4. The transferring licensee will receive notification from the LVS Help Desk of the verification outcome as soon as the verification is complete by the license issuing regulatory agency.

  5. The verification outcome will include a notification of whether:

    1. The requested materials, quantities, and authorized location are authorized on the license, or
    2. The requested materials, quantities, and/or authorized location are NOT authorized on the license, or
    3. Other reason (with an explanation)
  6. The licensee must keep a copy of the verification outcome sent by the LVS Help Desk for their records in accordance with the 10 CFR Part 37 requirements for keeping records of license verifications.
