Subject: 10CFR21 Interim Report: Potential Deficiencies In Enertech Commercial Grade Dedication Program

Dear Madam or Sir

In late 1999 Enertech manufactured and shipped electro-hydraulic actuators for installation on valves to be installed in an overseas plant Prior to shipment, the valve manufacturer observed pressure switch indication problems during final testing of the valves on which the actuators were installed.

Collective investigation by Burns and Roe Enertech, including its switch supplier and the valve manufacturer determined the problem resulted from changes in manufacturing methods implemented by the switch supplier. The immediate problem was addressed, the pressure switches were modified using the supplier's previous manufacturing methods, installed on the valves and subsequently passed all final Test requirements.

The noted change in manufacturing method was not detected by Enertech's commercial grade dedication process. Enertech initiated a corrective action review process to determine the root cause and impact of the potential deficiency. While Enertech's dedication process procedures are considered adequate by outside auditing agencies, Enertech has revised the appropriate procedures to strengthen requirements regarding sub-supplier design control. Enertech personnel involved in the dedication process have been trained regarding the events surrounding the incident discussed above, and on the enhanced requirements of the applicable Procedures.

To date, Enertech has determined The potential deficiency is limited to the case discussed above. However, the review is ongoing. Should Enertech determine any dedicated product previously supplied is at risk due to this potential deficiency, Enertech will make an immediate notification to the NRC, and the appropriate end-users.

Enertech is making this notification on the basis it has determined this issue to be potentially reportable under the provisions of 10CFR21 on October 22, 1999.

Enertech's evaluation of this issue under the provisions of 10CFR21 is not complete. Final evaluation results will be documented in a report to the NRC not later than November 30, 1999.
