Noncompliance, Section 21 (c) (3) (i), which requires "Initial
notification by facsimile", the enclosed initial notification is
provided. Written notification, in accordance with 10 CFR Part 21,
Section 21 (c) (3) (ii), will be provided by July 14, 1995.

II. IDENTIFICATION OF THE FACILITY, THE ACTIVITY, OR THE BASIC
COMPONENT SUPPLIED FOR SUCH FACILITY WHICH FAILS TO COMPLY OR
CONTAINS A DEFECT:
The facility is the R. E. Ginna Nuclear Power Plant. The basic
component is the Service Water outlet valve AOV-4561 (Fisher
Series 7600 valve) for the Containment Recirculation Fan
Coolers. This is an air-operated butterfly valve.
III. IDENTIFICATION OF THE FIRM CONSTRUCTING THE FACILITY OR
SUPPLYING THE BASIC COMPONENT WHICH FAILS TO COMPLY OR
CONTAINS
A DEFECT:
The butterfly valve was manufactured by:

NOTE: This valve was purchased commercial-grade and
commercially-dedicated by Rochester Gas & Electric
IV. NATURE OF THE DEFECT OR FAILURE TO COMPLY AND THE SAFETY
HAZARD
WHICH IS CREATED OR COULD BE CREATED BY SUCH DEFECT OR FAILURE
TO COMPLY:
The valve was supplied as an "air-to-open" valve instead of an
"air-to-close" valve. (Note: the original air-actuator was not
replaced.) The safety hazard that could be created by such a
defect is the loss of Service Water to the Containment
Recirculation Fan Coolers.

V. THE DATE ON WHICH THE INFORMATION OF SUCH DEFECT OR FAILURE TO
COMPLY WAS OBTAINED:
The information was obtained on April 18, 1995.
VI. IN THE CASE OF A BASIC COMPONENT WHICH CONTAINS A DEFECT OR
FAILS TO COMPLY, THE NUMBER AND LOCATION OF ALL SUCH
COMPONENTS
IN USE AT, SUPPLIED FOR, OR BEING SUPPLIED FOR GINNA STATION:
There was one valve (AOV-4561) procured, and this valve was
then installed in the Intermediate Building at Ginna Station.
VII. THE CORRECTIVE ACTION WHICH HAS BEEN, IS BEING, OR WILL BE
TAKEN; THE NAME OF THE INDIVIDUAL OR ORGANIZATION RESPONSIBLE
FOR THE ACTION; AND THE LENGTH OF TIME THAT HAS BEEN OR WILL BE
TAKEN TO COMPLETE THE ACTION:
Corrective action is complete. This action required altering
the actuator lever which resulted in the valve functioning
correctly. Rochester Gas & Electric maintenance department was
responsible for these actions.
VIII. ANY ADVICE RELATED TO THE DEFECT OR FAILURE TO COMPLY ABOUT
THE
FACILITY, ACTIVITY, OR BASIC COMPONENT THAT HAS BEEN, IS BEING,
OR WILL BE GIVEN TO PURCHASERS OR LICENSEES:
This valve was purchased as a replacement for a specific
numbered item. The original engineering requirements
stipulated "air-to-close" operation. The commercial-dedication
critical characteristics did not verify "air-to-close"
operation.
