AGREEMENT STATE REPORT - STUCK SHUTTERS
The following was received from the state of South Carolina via email:
"The South Carolina Department of Health and Environmental Control was notified via telephone on 10/31/22 that three fixed gauging device shutters were stuck in the closed position. All three fixed gauging devices are Thermo Fisher Scientific Model 5197 gauging devices, serial numbers B7842, B7847, and B7841. The activity of each gauging device is 100 mCi of Cs-137. The licensee is reporting that all three fixed gauging devices are mounted 12-15 feet above accessible areas. No elevated exposure rates are being reported. Department inspectors will be dispatched to the facility. This event is still under investigation by the South Carolina Department of Health and Environmental Control."

AGREEMENT STATE REPORT - LEAKING SEALED SOURCE
The following information was provided by the Washington State Department of Health via email:
"On 10/31/2022, a licensee, University of Washington - License Number: C001-1, reported that a 12.4 mCi Ni-63 source was leaking. The source was part of a Hewlett Packard electron capture detector (ECD) (Hewlett Packard, Model# G1223A, Source Serial # F5941). A pre-disposal wipe test of the ECD contained in a gas chromatograph (Hewlett Packard Model#5890) was performed on 10/25/22 after receiving the gas chromatograph from its owner for disposal. On 10/25/2022, the wipe test results indicated that the ECD had more than 0.005 microcuries of removable contamination. This device will be sent to NRD Static Control, LLC for recycling/disposal."

AGREEMENT STATE REPORT - FOUND MOISTURE DENSITY GAUGE
The following information was received via email from the Colorado Department of Public Health and Environment (the Department):
"During the afternoon of 10/31/22, an InstroTek model 3500 portable gauge was found by a member of the public and turned into Radiation Pros, LLC (specific license CO 1183-01). The isotopes are presumed to be 11 mCi cesium-137 and 44 mCi americium-241:beryllium, but the Department is awaiting confirmation. Radiation Pros, LLC contacted the Department to let us know that they have the InstroTek model 3500 on site and that they contacted InstroTek with the serial number of the portable gauge to find out who owned the gauge. InstroTek let Radiation Pros know that the gauge belongs to Geostruct Engineers, Inc. dba RMG-Rocky Mountain Group, radioactive material license number CO 758-01. Radiation Pros contacted Geostruct Engineers and let them know that they have the portable gauge. Geostruct Engineers picked up the InstroTek portable gauge and transported it back to the licensed location. This event appears to be unrelated to Colorado event CO210032 [EN 55510]."

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL
Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. NON-AGREEMENT STATE REPORT - UNABLE TO RETRACT RADIOGRAPHY CAMERA SOURCE
The following information was provided by the licensee via phone:
On November 5, 2022 at 2230 HST, Engineering & Inspections, Hawaii was performing radiography operations at a refinery using a QSA 880 Series Delta camera (Industrial Nuclear Model 7 source containing 84 Ci of Ir-192) when the source became stuck. Attempts to retrieve the source resulted in breaking the drive cable. The source remains in the guide tube. The licensee placed lead shielding around the source and is continuously monitoring the area. No overexposures were reported.
The licensee is attempting to contact Industrial Nuclear Corporation and QSA Global for assistance in retrieving the source.
