AGREEMENT STATE REPORT - LEAKING SEALED SOURCE

The following was received from the Illinois Emergency Management Agency (IEMA or the Agency) via email:

"On 10/12/2022, the Radiation Safety Officer [RSO] at Siemens Medical Solutions USA, Inc. (IL-01130-02), notified the Agency regarding a leaking Co-57 sealed source received on 10/12/2022 from the manufacturer, Eckert & Ziegler. The 15 mCi point source was opened and handled in a restricted area behind a small L-block. Upon removal from the packaging (noted as intact), the source was placed in a tungsten holder and was promptly leak tested by the RSO. Leak test results indicated contamination levels of 23,000 dpm [disintegrations per minute] on the source capsule and 140,000 dpm on the inside of the baggie in which the source was shipped in. Subsequent area and personal surveys were performed with no contamination noted. The source, baggie and a few tools used in the initial handling of the source were doubled bagged and remain secured at Siemens pending return to the manufacturer. The leaking source was reported to IEMA within 5 days as required in accordance with 32 Illinois Administrative Code 340.1260 and was then reported to the NRC as required. The investigation is ongoing."

AGREEMENT STATE REPORT - DAMAGED TROXLER GAUGE

The following information was provided by the New Jersey Department of Environmental Protection via email:

"On 10/14/2022, Soils Engineering Services Inc (SESI) notified New Jersey Department of Environmental Protection (NJDEP) that a moisture/density gauge (Troxler model 3430, serial number 22602 containing 9 mCi Cs-137 and 44 mCiAm-241/Be) was run over by a pickup truck at a jobsite in Bayonne, New Jersey. The case was crushed. The source was shielded when the gauge was run over. The licensee surveyed the gauge to confirm the source was properly shielded prior to transporting it to the licensee's storage location in Parsippany, NJ. A sealed source leak test is pending. The cause and corrective actions were equipment operator error. There was no release."

AGREEMENT STATE REPORT - STUCK OPEN SHUTTER

The following was received from Alabama Radiation Control via email:

"The licensed service provider arrived to address an issue with an Ohmart SHF2-0 source holder, serial number 8824GK with 500 millicuries of cesium-137 at assay date. The customer, Alabama River Cellulose, stated that the source holder was not functioning correctly (stuck open). The shutter issue was found during the site's lock-out/tag-out process for the process (unknown date/time). The service provider replaced the rotor on the device, and the device's shutter is now functioning as required."

EN Revision Text: SECONDARY CONTAINMENT INOPERABLE DURING SURVEILLANCE

The following information was provided by the licensee via email:

"At 0427 CDT on October 20, 2022, Grand Gulf Nuclear Station (GGNS) was in Mode 1 at 100 percent power when a failure of a draw down surveillance resulted in the loss of secondary containment. During the performance of the surveillance GGNS was unable to maintain secondary containment pressure, as required by Technical Specification Surveillance Requirement 3.6.4.1.4, greater than or equal to 0.266 inches of water vacuum for 1 hour at a flow rate of less than or equal to 4000cfm.

"The test was secured. Secondary containment was declared inoperable and Technical Specification 3.6.4.1 A.1 was entered at 0427 CDT. Secondary containment was restored to operable status at 0520 CDT by restoring the configuration to a previously known operable condition.

"This event is being reported as an event or condition that could have prevented the fulfillment of a safety function.

"The NRC Resident Inspector was notified."

TRANSPORT OF A POTENTIALLY CONTAMINATED PERSON OFFSITE

The following information was provided by the licensee via email:

"At 1446 EDT on October 20, 2022, an individual was transported offsite for treatment at an offsite medical facility. Due to the nature of the medical condition, the individual was not thoroughly surveyed prior to being transported offsite. Follow-up surveys performed by radiation protection technicians identified no contamination of the worker or of the ambulance and response personnel.

"This is an eight-hour notification, non-emergency for the transportation of a contaminated person offsite. This event is reportable in accordance with 10 CFR 50.72(b)(3)(xii).

"The NRC Resident Inspector has been notified."
