EN Revision Text: AGREEMENT STATE REPORT - STUCK / BROKEN SHUTTER

The following report was report was received from the Pennsylvania Department of Bureau Radiation Protection via email:

"On February 9, 2022, the licensee's radiation safety officer (RSO) was completing shutter checks and leak tests on a Ronan, Model Number: SAI-F37 fixed gauge. Its serial Number is AA-485 and it contains 10 milliCuries of Cesium-137. During the checks the shutter's shear pin broke and he was unable to close the shutter. The vessel that this gauge is on is not entered very often and is not readily accessible. The RSO has notified the manufacturer for a repair. No more information is available at this time."

EN Revision Text: REPORTABLE OCCURANCE RELATED TO BUILDING EXHAUST FAN

The following information was provided by the licensee via fax or email:

"While performing the pre-start checkout on 2022-02-15 [at approximately 0800 EST], the SRO [Senior Reactor Operator] performing the checkout discovered that the building exhaust fan was not operating. This fan is required to be operating during reactor operations, and it is typically left running all the time. Troubleshooting determined that the cause of the fan loss was power loss from a conduit being cut the previous day when a small area of the cement floor in the janitor closet was being removed for replacement of the janitor closet sink. The concrete work was performed between 1200 and 1453 EST, which was when the contractor signed in and out. The reactor was shut down at 1551 EST, so the reactor was operated for at least an hour without the fan operating.

"There were no safety implications for the reactor, the staff, or the public."

EN Revision Text: LOSS OF OFFSITE COMMUNICATION CAPABILITY

The following information was provided by the licensee via fax or email:

"At 1159 EST, on 2/16/2022, the Watts Bar Nuclear, Shift Manager was notified that Tennessee Valley Authority (TVA) attempted to notify Tennessee Emergency Management Agency (TEMA) regarding routine siren testing at 0750 EST. TVA was unable to reach TEMA via telephone land line or the Emergency Communication and Notification System (ECNS). TEMA Watch Point staff were located at their back-up facility. TVA subsequently notified TEMA via cell phone that there were communication issues with the primary and backup notification methods. It was determined that the TEMA back-up facility was not able to receive incoming calls. At 0820 EST, TEMA positioned personnel at their primary facility in order to respond to notifications. This restored primary and backup means of notifying the state because the primary facility was not affected by the communication issues.

"This event is reportable in accordance with 10 CFR 50.72(b)(3)(xiii) as a Major Loss of Offsite Communications Capability because it affected TVA's ability to notify the State of TN.

"The licensee has notified the NRC Resident Inspector."
