The following information was received by the State of Ohio via email:
"The licensee performs prostate seed implants on Tuesdays and Thursdays each week. End of day inventories are performed by two therapists. At the end of day inventory on 1/4/2022 the licensee had 53 unused seeds. The beginning of day inventory on 1/6/2022 the inventory was 52 seeds. No procedures were conducted between 1/4/22 and 1/6/22, and surveys did not locate the missing seed. The licensee is unable to account for the discrepancy in their inventory."

The following information was obtained from the Arkansas Department of Health (Department) via email:
"On January 7, 2022, at 1000 (CST), the licensee reported the discovery (same day) of a damaged tritium exit sign. During preparations for removal of all tritium exit signs at the facility one was discovered to have been subjected to heat. The face of the exit sign was damaged, it is not known if any damage is below the face plate.
"The licensee is currently working with a consultant on the removal of the exit sign.
"The exit sign is identified as: Evenlite Model 201, Serial Number 1550F, containing 10.5 Curies of H-3.
"The Department will monitor the safe removal and disposal of the exit sign and will update this report."

EN Revision Text: AGREEMENT STATE REPORT - FOUND MOISTURE DENSITY GUAGE
The following is a synopsis of an email received from the State of Florida:
The State of Florida received a call today around 1530 EST from a business to report they found a Humboldt Soil Moisture Density Gauge [Model 5001P, Serial number 547, 1988] in the gravel parking lot of their place of business. There was only a gauge without it's case. The gauge was found in an upright position, with the source rod appearing to be in the retracted position. The gauge did not appear to damaged, but does look to be very used and worn. The Tampa Bureau of Radiation Control Inspector was notified and is in route to investigate, secure the gauge, and transport it to the Tampa storage unit until it can be transported to the ERCE office in Orlando. The Tampa Police Department was also notified.
Humboldt literature from the internet shows the sources to be Cesium-137 less than 11 millicuries and Americium-241/Be less than 44 millicuries.

OFFSITE NOTIFICATION DUE TO SPURIOUS SIREN ACTUATION
The following information was provided by the licensee via fax:
"On 1/13/2022 at 0806 CST, Nebraska Public Power District was notified by Atchison County Missouri of a spurious actuation of [Cooper Nuclear Station] (CNS) Emergency Siren 2113 near Rockport, Missouri from approximately 0800 to 0805 CST.
"Nebraska Public Power District will issue a press release for this event. The CNS Emergency Alert System (EAS) was not activated.
"This condition is reportable under 10 CFR 50.72(b)(2)(xi) for any event or situation for which a news release is planned or notification to other government agencies has been or will be made which is related to heightened public or government concern.
"The NRC Senior Resident Inspector has been informed."
