| DISCOVERY OF A MISSING M-58 AIMING POST LIGHT SOURCE CONTAINING |
| APPROXIMATELY 5 Ci OF TRITIUM |
| |
| On 12/10/99, the U.S. Army in Rock Island, Illinois, was notified that an |
| M-58 aiming post light source was missing from a U.S. Army facility in Ft. |
| Drum, New York. The missing source contained approximately 5 Ci of tritium, |
| and the source had been removed from the aiming post light. Because there |
| are typically no maintenance activities associated with aiming post lights, |
| it is currently believed that the source was stolen. The source was |
| discovered to be missing on approximately 12/06/99. |

| OUTSIDE DESIGN BASIS - BOTH TRAINS OF CONTROL ROOM AIR CONDITIONING |
| INOPERABLE |
| |
| "At 0710 [CST], while attempting to rotate running equipment, the operator |
| placed the control switch for the B Train control Room A/C to ON. |
| Subsequently, the A Train switch was placed to OFF to allow the B Train to |
| start. The B Train failed to start, and the A Train switch was placed back |
| to AUTO. Further investigation determined that the A Train switch was |
| placed to OFF greater than 20 seconds following the B switch being placed to |
| ON. This resulted in a Trip signal being generated for the B Train unit. |
| Therefore, both trains of Control Room A/C were out of service with the B |
| Train Trip signal and the A Train switch in the OFF position. This |
| condition existed for a total of 12 seconds before the A Train unit was |
| restarted. The need to report this condition was not identified until 1025 |
| [CST]." |

| AGREEMENT STATE REPORT - LOST P32 |
| |
| "On December 16, 1999, the Agency was notified by the Licensee that a vial |
| of P-32 labeled ATP was missing from the Department of Microbiology. The |
| vial, the inner package, of a shipping container was placed in a storage |
| freezer on November 17, 1999. The vial contained 0.5 millicuries on November |
| 17, 1999. As of December 16, 1999, it contained 0.123 millicuries. An |
| initial search by the Licensee failed to locate the missing vial or its |
| contents. The initial search included surveys of the research laboratory in |
| which the material was stored. The loss was detected on December 15, 1999, |
| when a technician prepared to use the material. |

| INITIATED REGARDING A MINOR OUT-GASSING AT THE X-343 FEED VAPORIZATION AND |
| SAMPLING FACILITY WHILE SAMPLING A DOE CYLINDER IN AUTOCLAVE SIX. RESPONSE |
| PERSONNEL STOPPED THE OUT-GASSING AND STABILIZED THE CYLINDER FOR COOLDOWN. |
| THIS EVENT IS REPORTABLE DUE TO NOTIFICATION BEING MADE TO AN OUTSIDE AGENCY |
