| HIGH PRESSURE COOLANT INJECTION (HPCI) SYSTEM DECLARED INOPERABLE DUE TO     |
| FAILURE OF THE ASSOCIATED ROOM COOLER                                        |

| "DURING NORMAL OPERATIONS, HPCI HAS BEEN DECLARED INOPERABLE IN ACCORDANCE   |
| WITH TECH SPEC SECTION 3.5.A.3.g.  HPCI IS INOPERABLE DUE TO THE FAILURE OF  |
| SUPPORT AUXILIARY EQUIPMENT, i.e., THE VENTILATION COOLING COIL HAS A MAJOR  |
| LEAK REQUIRING ISOLATION OF THE VENTILATION UNIT.  HPCI IS IN A 14-DAY       |
| LCO."                                                                        |

| "This notification retracts a notification made 11/7/99 due to HPCI being    |
| declared inoperable for failure of required auxiliary support equipment.     |
| V-AC-8B is one of two room coolers in the HPCI room.  V-AC-8B, considered    |
| required auxiliary support equipment, was isolated due to a coil leak.       |
|                                                                              |
| "Further analysis shows that [the] HPCI building room would remain under     |
| it's design temperature and operable with no room coolers operable.          |
| Investigation following the event determined that the leaking water did not  |
| have any effect on HPCI instruments or operability.  The leak was repaired,  |
| and the system [was] returned to normal standby condition."                  |
|                                                                              |
| The licensee intends to notify the NRC Resident Inspector.  The Headquarters |
| Operations Officer notified the R3DO (Patrick Hiland).                       |

| A contract supervisor has tested positive for illegal drug use during a      |
| random fitness-for-duty test.  The employee's access into the plant          |
| protected area has been suspended.  Refer to the HOO Log for additional      |

| OFFSITE NOTIFICATION REGARDING THE DISCOVERY OF A DEAD FLORIDA CROCODILE (AN |
| ENDANGERED SPECIES) IN THE SITE'S COOLING CANAL SYSTEM                       |
|                                                                              |
| At 1320 on 12/07/99, the Land Utilization Group notified the control room of |
| the discovery of a dead crocodile in the site's cooling canal system.  It is |
| currently believed that the dead crocodile lost a fight with a larger        |
| crocodile.                                                                   |
|                                                                              |
| The licensee notified the NRC resident inspector.  In accordance with an     |
| Environmental Permit, the licensee also plans to notify the Florida Game and |
| Freshwater Fish Commission.                                                  |

| INOPERABILITY OF HPCI DUE TO A LOOSE PIPE SUPPORT                            |
|                                                                              |
| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "During normal plant operation the High Pressure Core Injection (HPCI)       |
| system was declared inoperable [in accordance with] Technical Specification  |
| section 3.5.A.3.g.  HPCI is inoperable due to an inoperable pipe support on  |
| the steam supply to the HPCI drive turbine.  The support was observed to be  |
| loose, and the anchor bolts on the base of the support are pulled out of the |
| wall approximately 3/8".  HPCI is in a 14-day [limiting condition for        |
| operation]."                                                                 |
|                                                                              |
| The licensee notified the NRC resident inspector.                            |
