| THREE SPRINKLER SYSTEMS DECLARED INOPERABLE DUE TO CORRODED HEADS (24-hour |
| report) |
| |
| The following text is a portion of a facsimile received from Paducah: |
| |
| "On 06/02/99 at 1630 CDT, the Plant Shift Superintendent (PSS) was notified |
| that numerous sprinkler heads were corroded, affecting 16 sprinkler systems |
| in C-337 and one system in C-333, such that the ability of the sprinklers to |
| flow sufficient water was called into question. Subsequently, these |
| sprinkler systems were declared inoperable, and TSR-required actions |
| establishing roving fire patrols were initiated. This deficiency was |
| detected during scheduled system inspections conducted by Fire Protection |
| personnel. Currently, functionality of the sprinkler heads has not been |
| fully evaluated by Fire Protection personnel, and the remaining cascade |
| buildings are currently being inspected, and if necessary, this report will |
| be updated to identify any additional areas. |

| "Two sprinkler heads on system D-1 in C-337 and two sprinkler heads on |
| system 27 in C-335 were identified to also be corroded. These were |
| identified to the PSS on 06/03/99 at 1600 CDT and 1601 CDT, respectively, |
| and determined to require an update to this report by the PSS. |
| |
| "It has been determined that this event is reportable under |
| 10CFR76.120(c)(2) as an event in which equipment is disabled or fails to |
| function as designed." |

| Paducah personnel notified the NRC resident inspector of this update. The |
| NRC operations officer notified the R3DO (Hills) and NMSS EO (Combs). |

| C-333 were identified to have corrosion. The PSS was notified of this |
| condition at 1350 CDT on 06/18/99. The area of the fire patrol for system |
| C-15 was expanded to include the two heads identified as corroded. The one |
| head on system B-8 was in the area already being patrolled. The PSS |
| determined that an update to this report was required." |

| in C-337 were identified to have corrosion. The PSS was notified of the |
| condition on system D-8 at 0125 CDT on 06/25/99 and at 1019 CDT on 06/25/99 |
| for system D-7. Both systems were immediately declared inoperable and LCO |
| fire patrol actions were implemented. It was determined that an update to |
| this report was required." |

| determined that an update to this event report was required." |

| The following information was provided as an update to this report: |

| "Two adjacent sprinkler heads on C-337 System D-15 were identified to have |
| corrosion and the PSS declared the system inoperable at 0925 [CDT] on |
| 8/29/99. Also two adjacent heads on C-337 System D-10 were identified to |
| have corrosion and the PSS declared the system inoperable at 1450. LCO |
| required fire patrols for both of the affected areas were initiated in the |
| time frame required by TSR. The PSS determined that an update to this |
| report was required." |

| inoperable at 2155 CDT on 9/7/99. An LCO required fire patrol for the |
| affected area was initiated in the time frame required by the TSR. Paducah |
| personnel informed the NRC Resident Inspector. The NRC operations officer |
| notified the R3DO (Monte Phillips) and NMSS (Josie Piccone). |

| UNUSUAL EVENT DECLARED DUE TO A FREON LEAK FROM A CONTROL ROOM CHILLER INTO |
| THE AUXILIARY BUILDING |
| |
| The licensee declared the unusual event at 2133 on 09/29/99 due to a release |
| of toxic gas (a Freon leak) deemed to be detrimental to safe operation of |
| the plant. No radiological release is in progress. The 33' wind direction |
| is from 146� at 17.9 miles per hour. There were no protective actions |
| recommended. |
| |
| At 2117 hours on 09/29/99, the 'B' control room ventilation train chiller |
| tripped resulting in an automatic start of the 'A' control room ventilation |
| train. The trip was the direct result of a major freon leak dispersing into |
| the auxiliary building. All non-essential personnel were evacuated from the |
| auxiliary building. An Unusual Event was declared at 2133 hours on 09/29/99 |
| in accordance with Emergency Classification Guidance Section 9.4.1.B, |
| release of toxic gas deemed detrimental to safe operation of the plant. No |
| personnel injuries have occurred, and all access to the auxiliary building |
| is being controlled until follow-up actions are complete. |
| |
| The 'A' control room vent train is currently in service. There is no |
| indication of any freon in the control room, and the control room was not |
| placed on recirculation. |
| |
| The licensee plans to terminate the Unusual Event when the freon leak is |
| isolated and when air levels in the auxiliary building are restored to |
| normal. |
| |
| The licensee stated that the emergency response data system (ERDS) has not |
| been activated because ERDS is down for the performance of Y2K upgrades. |
| |
| The licensee notified the NRC resident inspector, the states of Delaware and |
| New Jersey, and the Lower Alloways Creek Township. |
| |

| |
| The leak was terminated because the full freon charge in the chiller |
| dispersed (2,200 pounds), and the atmosphere in the auxiliary building has |
| been restored to normal. As a result, the licensee terminated the Unusual |
| Event at 0158 hours on 09/30/99. |

| The licensee notified the NRC resident inspector, the states of Delaware and |
| New Jersey, and the Lower Alloways Creek Township. The NRC operations |

| INADVERTENT START OF THE "B" RESIDUAL HEAT REMOVAL (RHR) PUMP DURING |
| MAINTENANCE ACTIVITIES |
| |
| Electrical maintenance was removing a spare breaker in the "B" train 480 |
| volt switchgear (XSW-1DB1). This cubicle is directly above the breaker |
| cubicle for the "B" RHR pump breaker. During the maintenance activity, a |
| breaker transport cart inadvertently rolled against the supply breaker for |
| the "B" RHR pump. The lip of the cart activated the manual closing |
| mechanism, on the front of the RHR pump breaker. This resulted in starting |
| the "B" RHR pump. The pump ran for approximately six minutes before being |
| secured, and placed in the pull-to-lock (PTL) position after it was verified |
| that no valid demand signal was present. The pump remained in PTL for 29 |
| minutes while the investigation continued. |

| At 1906 EDT on 9/30/99 a reactor scram occurred due to a turbine trip and |
| generator lockout. During the transient three safety relief valves opened |
| momentarily due to high reactor pressure and then reseated. Reactor |
| pressure peaked at approximately 1125 psi. Additionally Group II and III |
| Primary Containment Isolation System (PCIS) isolations occurred due to low |
| reactor water level following the scram. Minimum reactor level following |
| the scram was approximately -15 inches. All control rods fully inserted. |
| Post scram recovery was normal and all scram and PCIS signals have been |
| reset. The licensee is investigating the cause of the reactor |
| trip/generator lockout. |

| RHR PUMP DISCHARGE VALVE FAILED TO CLOSE ON DEMAND |

| The #12 Residual Heat Removal (RHR) pump motor operated discharge valve |
| failed to close upon demand. This valve is required by Emergency Operating |
| Procedures to be closed during cold leg recirculation to provide long term |
| containment spray. The cause of the failure is currently under |
| investigation. |
| |
| The licensee will inform the NRC resident inspector and LAC Township. |

| EMERGENCY FEEDWATER START |

| The unit was in the process of a normal plant shut down for a refueling |
| outage when the running Main Feedwater pump (MFP-2A) tripped. The tripping |
| of the only running feedwater pump caused Emergency Feedwater to actuate |
| (EFP-1 and EFP-2). The plant did not experience any abnormal changes in |
| OTSG level or other systems. The cause of the MFP trip is unknown at this |
| time. The "B" MFP (MFP-2B) was started and emergency feedwater secured and |
| reset. |

| The licensee informed the NRC resident inspector. |
