| THREE SPRINKLER SYSTEMS DECLARED INOPERABLE DUE TO CORRODED HEADS (24-hour   |
| report)                                                                      |
|                                                                              |
| The following text is a portion of a facsimile received from Paducah:        |
|                                                                              |
| "On 06/02/99 at 1630 CDT, the Plant Shift Superintendent (PSS) was notified  |
| that numerous sprinkler heads were corroded, affecting 16 sprinkler systems  |
| in C-337 and one system in C-333, such that the ability of the sprinklers to |
| flow sufficient water was called into question.  Subsequently, these         |
| sprinkler systems were declared inoperable, and TSR-required actions         |
| establishing roving fire patrols were initiated.  This deficiency was        |
| detected during scheduled system inspections conducted by Fire Protection    |
| personnel.  Currently, functionality of the sprinkler heads has not been     |
| fully evaluated by Fire Protection personnel, and the remaining cascade      |
| buildings are currently being inspected, and if necessary, this report will  |
| be updated to identify any additional areas.                                 |

| 10CFR76.120(c)(2) as an event in which equipment is disabled or fails to     |
| function as designed."                                                       |

| "Two sprinkler heads on system D-1 in C-337 and two sprinkler heads on       |
| system 27 in C-335 were identified to also be corroded.  These were          |
| identified to the PSS on 06/03/99 at 1600 CDT and 1601 CDT, respectively,    |
| and determined to require an update to this report by the PSS.               |

| 10CFR76.120(c)(2) as an event in which equipment is disabled or fails to     |
| function as designed."                                                       |

| Paducah personnel notified the NRC resident inspector of this update.  The   |
| NRC operations officer notified the R3DO (Hills) and NMSS EO (Combs).        |

| "Three sprinkler heads on system C-15 and five sprinkler heads on system B-8 |
| in C-333 were identified to also be corroded.  The PSS was notified of this  |
| condition at 1300 CDT on 06/17/99 and determined that an update to this      |
| report was required."                                                        |

| "It has been determined that this event is reportable under                  |
| 10CFR76.120(c)(2) as an event in which equipment is disabled or fails to     |
| function as designed."                                                       |

| Paducah personnel notified the NRC resident inspector of this update.  The   |
| NRC operations officer notified the R3DO (Madera).                           |

|                                                                              |
| "Two sprinkler heads on system C-15 and one sprinkler head on system B-8 in  |
| C-333 were identified to have corrosion.  The PSS was notified of this       |
| condition at 1350 CDT on 06/18/99.  The area of the fire patrol for system   |
| C-15 was expanded to include the two heads identified as corroded.  The one  |
| head on system B-8 was in the area already being patrolled.  The PSS         |
| determined that an update to this report was required."                      |

| "Two sprinkler heads on system D-8 and three sprinkler heads on system D-7   |
| in C-337 were identified to have corrosion.  The PSS was notified of the     |
| condition on system D-8 at 0125 CDT on 06/25/99 and at 1019 CDT on 06/25/99  |
| for system D-7.  Both systems were immediately declared inoperable and LCO   |
| fire patrol actions were implemented.  It was determined that an update to   |
| this report was required."                                                   |

| "Five sprinkler heads and one sprinkler piping tee on C-337 system D-7 were  |
| identified to have corrosion. The PSS was notified of these corroded system  |
| parts and declared the system inoperable at 0931 CDT on 07/30/99.  LCO       |
| required fire patrols of the affected area were initiated.  The PSS          |
| determined that an update to this event report was required."                |

| "Two sprinkler heads on C-337 System D-1 were identified to have corrosion.  |
| The PSS was notified of these corroded system parts and declared the system  |
| inoperable at 1155 CDT on 7/31/99.  LCO required fire patrols of the         |
| affected area were initiated.  The PSS determined that an update to this     |
| report was required."                                                        |

| inoperable at 1130 CDT on 8/10/99.  LCO required fire patrols of the         |
| affected area were initiated.  The PSS determined that an update to this     |
| report was required."                                                        |

| the system inoperable at 0918 CDT on 8/22/99.  LCO required fire patrols     |
| were initiated.  The PSS determined that an update to this report was        |
| required.                                                                    |

| "Two adjacent sprinkler heads on C-337 System D-11 were identified to have   |
| corrosion.  The PSS was notified of these corroded system parts and declared |
| the system inoperable at 1420 [CDT] on 8/28/99.   LCO required fire patrols  |
| of the affected area were initiated. The PSS determined that an update to    |
| this report was required."                                                   |

| The following information was provided as an update to this report:          |
|                                                                              |
| "Two adjacent sprinkler heads on C-337 System D-15 were identified to have   |
| corrosion and the PSS declared the system inoperable at 0925 [CDT] on        |
| 8/29/99.  Also two adjacent heads on C-337 System D-10 were identified to    |
| have corrosion and the PSS declared the system inoperable at 1450.   LCO     |
| required fire patrols for both of the affected areas were initiated in the   |
| time frame required by TSR.  The PSS determined that an update to this       |
| report was required."                                                        |

| FAILURE OF 13 HIGH PRESSURE FIRE WATER SPRINKLER SYSTEMS TO MEET SYSTEM      |
| OPERABILITY REQUIREMENTS  (24-Hour Report)                                   |
|                                                                              |
| The following text is a portion of a facsimile received from Portsmouth:     |
|                                                                              |
| "On 08/10/99 at approximately 1600 hours, the Plant Shift Superintendent     |
| (PSS) was notified that 13 sprinkler systems associated with the High        |
| Pressure Fire Water (HPFW) system were not capable of meeting system         |
| operability requirements.  This condition was discovered as a result of an   |
| engineering review of individual sprinkler heads that had previously been    |
| identified as exhibiting corrosion around the valve seat.  The engineering   |
| review was conducted following the discovery that similar corrosion problems |
| experienced at the Paducah Gaseous Diffusion Plant resulted in sprinkler     |
| heads being unable to actuate at normal pressure.  An initial screening of   |
| PORTS sprinkler heads known to exhibit corrosion was conducted, and systems  |
| where the potential for two or more adjacent heads to be corroded were       |
| inspected.  This identified 11 systems in the X-333 and 2 systems in the     |
| X-326 that did not meet the sprinkler system operability requirement that no |
| two adjacent sprinkler heads be inoperable."                                 |
|                                                                              |
| "The PSS declared the affected sprinkler systems inoperable, and TSR         |
| required actions were completed.  Further inspections of sprinkler systems   |
| are planned.  This report will be updated if additional sprinkler systems    |
| are determined to be inoperable."                                            |
|                                                                              |
| "There was no loss of hazardous/radioactive material or                      |
| radioactive/radiological contamination  exposure as a result of this         |
| event."                                                                      |
|                                                                              |
| Portsmouth personnel notified the NRC resident inspector as well as the      |
| onsite Department of Energy representative.                                  |
|                                                                              |
| *** UPDATE ON 8/19/99 AT 2327 FROM HALCOMB TO POERTNER ***                   |
|                                                                              |
| During continuing walkdowns, HPFW system #398 was discovered with two or     |
| more (3) adjacent sprinkler heads with mineral deposits.  This HPFW          |
| sprinkler was declared inoperable at 1038 hours and was repaired at 1407     |
| hours.                                                                       |
|                                                                              |
| Portsmouth personnel notified the NRC resident inspector.                    |

| The following information was provided as an update to this report:          |
|                                                                              |
| "During continuing walk downs of the HPFW system, systems 194, 196, 197,     |
| 211, 212 were found with two or more adjacent sprinkler heads unable to meet |
| system operability.  Systems were made inoperable at 1452 on 8/27/99.  All   |
| sprinklers were repaired and system operability made at 0550 8/26/99."       |

| At 0030 CDT on 08/27/99, it was determined that a security event which       |
| occurred at 2230 CDT on 08/26/99 met the reportability requirements of 10    |
| CFR 73.71(b), Part 73, Appendix G, paragraph 1(c).  The event involved a     |
| safeguards system degradation related to power supply functions.             |
| Compensatory measures were taken upon discovery.  (Contact the NRC           |
| operations officer for additional details and for a site contact telephone   |
| number.)                                                                     |

| EMERGENCY SERVICE WATER SYSTEM DEGRADATION (ONLY ONE OF FOUR PUMPS           |
| OPERABLE)                                                                    |
|                                                                              |
| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "On 08/26/99, Susquehanna Unit 1 and Unit 2 were operating at 100% power     |
| with the 'B' loop of emergency service water [(ESW)] out of service for      |
| scheduled maintenance.  During testing on the ESW system, with all ESW pumps |
| in service, it was identified that the 'C' and 'D' ESW pumps' discharge      |
| check valves were closed.  The ESW flow surveillance was performed, and the  |
| 'C' and 'D' ESW pumps failed to achieve the required flow and were declared  |
| inoperable.  Concurrently, the 'B' loop of ESW was returned to service.      |
| During the time the 'B' ESW loop was inoperable, the 'A' ESW pump was the    |
| only one operable ESW pump.  This constitutes a serious degradation of the   |
| plant in that it is a condition which is outside of design basis and,        |
| therefore, reportable under 10 CFR 50.72(b)(1)(ii) requiring a 1-hour ENS    |
| notification."                                                               |

| AGREEMENT STATE REPORT INVOLVING RAD LEVEL EXCEEDING STATE LEVELS DURING     |
| SHIPMENT                                                                     |
|                                                                              |
| "This notification is being sent to you per instructions in the NRC Event    |
| Reporting Handbook. This event does not pose a threat to public health and   |
| safety but is reportable under 1200-2-5-.16(2)(d) of SRPAR.  This incident   |
| occurred on August 4, 1999.  Please see the attached report for derails      |
| involving this event.                                                        |
|                                                                              |
| "This letter is sent concerning the American Ecology Recycle Center d/b/a    |
| U.S. Ecology Nuclear Materials Management Center (NMMC) Radioactive Material |
| License R-01037-B04. The NMMC is providing written documentation of the      |
| reportable event that was reported to your office via telecom on August 6,   |
| 1999.  The event  involved receipt of a radioactive material shipment with   |
| external radiation levels in excess of those reportable under State          |
| Regulations for Protection Against Radiation (SRPAR) Section                 |
| 1200-2-5-.16(2)(d).                                                          |
|                                                                              |
| "Investigation:                                                              |
|                                                                              |
| "The subject event involved the U.S. Ecology Brokerage division truck and    |
| driver on-loading a shipment consisting of a source drum at CTL Engineering, |
| Columbus, OH.   Receipt surveys performed at the NMMC denoted an on-contact  |
| reading of 450 mR per hour (i.e., greater than the reportable 200 mR per     |
| hour) at a single point on the bottom of the truck.  The location of the     |
| reportable radiation level was not such that it would provide a credible     |
| exposure opportunity for a member of the public.                             |
|                                                                              |
| "After on-loading the subject source drum, the truck made eight more stops   |
| throughout the mid-west (i.e., OH, MI, IL, IN) over a period of 7 days, with |
| each stop ranging in duration between approximately � hour and 4 hours per   |
| stop.  Interviews with the driver indicate that each stop was in an          |
| industrial location with no notable personnel activity (i.e., occupancy)     |
| around the truck. The driver also noted that he makes a conscious effort to  |
| park away from others when making nightly stops. Therefore, he did not note  |
| any personnel activity around his truck that could have led to a credible    |
| exposure opportunity for a member of the public.                             |
|                                                                              |
| "Safety Significance:                                                        |
|                                                                              |
| "There is no plausible scenario in which a member of the public was          |
| over-exposed due to this event, The driver, who was the maximally exposed    |
| individual, was issued DOSIMETRY of record at the time of the event.  His    |
| record dose will be evaluated upon the subject TLD being processed.          |
|                                                                              |
| "Cause:                                                                      |
|                                                                              |
| "The most plausible cause for the event was that the driver made a human     |
| performance error while reading his survey meter. Contributing causes to     |
| this error were: (1) the infrequency with which the driver works with        |
| material involving higher radiation levels and (2) the infrequency with      |
| which the driver's radiological skills training is reinforced.               |
|                                                                              |
| "Corrective Actions:                                                         |
|                                                                              |
| "The source drum was properly handled upon receipt at the NMMC.              |
|                                                                              |
| "The driver's radiological skills were upgraded through training.            |
|                                                                              |
| "The driver will receive periodic radiological skills evaluations as needed  |
| to maintain the adequate level of proficiency.                               |
|                                                                              |
| "Safety Significance:                                                        |

| PART 21 NOTIFICATION INVOLVING UNACCEPTABLE RETENTION CAPABILITY FOR BNC     |
| CONNECTORS                                                                   |
|                                                                              |
| "In accordance with the requirements for basic components regulated by the   |
| Nuclear Regulatory Commission, as specified in 10 CFR 21.21, I am providing  |
| this notification.  In June 1999, the reliability engineer with the 'systems |
| group' department of our company sent me a memo that suggested we might have |
| accepted an unacceptable part for the 30-4 connector, which is a regulated   |
| component.  He had connected an AMP BNC connector to a cable to determine    |
| its equivalency to the other qualified part from Amphenol p/n 31-2.  He      |
| found that when the wires were combed out as directed on the instructions    |
| that came with the AMP connector, it did not appear to perform correctly     |
| with respect to cable retention, and [he] asked if someone in QA could       |
| verify his preliminary testing.                                              |
|                                                                              |
| "Subsequently, our Quality Manager examined the connector and obtained       |
| instructions from the manufacturer that said the wires should be left        |
| braided, not combed out, and that if you left it braided, a stronger grip    |
| was achieved.  The reliability engineer tried the braided connection and did |
| find that the retention was improved.  However, in investigating the matter  |
| further with the manufacturer, it was found that the manufacturer did have   |
| conflicting instructions but said the drawing specifying a braided           |
| connection should be considered the official one since it was a controlled   |
| document.  It was also found that the part from AMP was not the originally   |
| qualified part number - 221552-1.  AMP had obsoleted that part in 1992 and   |
| substituted the 221265-0 in its place.  The new part, however, did not carry |
| a military specification for shock (MIL-Std. 202) and was not military type  |
| UG-88 like the previous one - conditions which provided assurance that it    |
| met certain criteria needed for Seismic Class 1 rating.  This information    |
| was presented at the company's Quality Review Meeting on August 12, 1999.  A |
| technical evaluation was conducted and reported in an abstract for the 30-4  |
| BNC Connector by the reliability engineer on August 26, 1999.                |
|                                                                              |
| "The connectors for part 30-4 that were in stock were inspected, and it was  |
| found that all of them were this incorrect AMP part number.  The connectors  |
| were subsequently put on hold by the Quality Manager, and correct connectors |
| have been ordered from Amphenol.                                             |
|                                                                              |
| "One hundred pieces of the incorrect part (AMP 1-221265) were received at    |
| Victoreen (now a part of Inovision) in January 1997.  A search has been      |
| conducted to identify those parts, assemblies, and kits which list part 30-4 |
| as part of their bill of material.  From those, a review will be done to     |
| identify those that have been produced since the receipt of the incorrect    |
| part and, therefore, have the potential for being incorrect.  The customers  |
| who have received the identified products will be sent a notification within |
| 30 days.  If acceptable replacement parts can be obtained with that time     |
| frame, they will be sent with the notification.                              |
|                                                                              |
| "A notification was sent to the Plant Manager/Senior Executive and to the    |
| Chief Operating Officer of Inovision on August 26, 1999.  The Plant Manager  |
| has authorized me to submit this report by facsimile to the NRC Operations   |
| Center.  A written report containing the information specified at 10 CFR     |
| 21.21(d)(4) will be submitted to the NRC within 30 days."                    |

| THE LICENSEE PLANS TO NOTIFY THE STATE OF NORTH CAROLINA THAT THEY MAY HAVE  |
| VIOLATED THEIR NPDES (NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM)       |
| PERMIT.  (Refer to event  numbers 36084 and 36086 for related events.)       |

| PLANT OPERATORS OPENED THE SITE STORM DRAIN COLLECTOR BASIN DISCHARGE VALVES |
| TO THE CIRCULATING WATER DISCHARGE CANAL.  THESE VALVES BYPASS THE NORMAL    |
| PATH TO THE STABILIZATION POND AND CONSTITUTE A POTENTIAL UNMONITORED        |
| RADIOACTIVE LIQUID RELEASE.  THESE VALVES WERE OPENED DUE TO THE EXCESSIVE   |
| RAIN ASSOCIATED WITH HURRICANE DENNIS, AND THEY WILL BE CLOSED WHEN THE RAIN |
| SUBSIDES.  THIS CONDITION REQUIRES THE LICENSEE TO NOTIFY THE STATE BECAUSE  |
| OF THEIR NPDES PERMIT.                                                       |
