| PART 21 REPORT -  DEVIATION IN A NUCLEAR LOGISTICS INC (NLI) DC POWER        |
| SUPPLY.                                                                      |

| A DEVIATION HAS BEEN IDENTIFIED IN A NLI DC POWER SUPPLY, P/N                |
| NLI-LGS-G-24-OV-R-8018 WHICH HAVE BEEN SUPPLIED AS A CLASS 1E COMPONENT,     |
| THAT ALLOWS A PREMATURE OVER VOLTAGE PROTECTION ACTUATION TO OCCUR IN A      |
| CIRCUIT SPECIFIC APPLICATION.  THIS CONDITION OCCURRED DURING DIESEL         |
| GENERATOR STARTUP, SPECIFICALLY DURING DIESEL GENERATOR LOADING.  THIS ISSUE |
| WAS DETERMINED TO BE A DEVIATION ON 4/19/99, HOWEVER, TO DATE NLI HAS NOT    |
| BEEN ABLE TO DUPLICATE THIS CONDITION DURING LABORATORY TESTING.  DATA       |
| COLLECTION AND ADDITIONAL TESTING IS BEING PERFORMED TO RESOLVE THE          |
| DEVIATION.  IT THE PREMATURE OVER VOLTAGE PROTECTION ACTUATION IS DETERMINED |
| TO BE THE RESULT OF THE POWER SUPPLY DESIGN, OTHER NLI-L SERIES POWER        |
| SUPPLIES WILL BE TESTED TO DETERMINE THEIR SUSCEPTIBILITY TO SIMILAR         |
| CONDITIONS.  TESTING AND EVALUATION ACTIVITIES WILL BE COMPLETED ON 7/16/99. |
| THESE COMPONENTS HAVE BEEN SUPPLIED AS SAFETY-RELATED COMPONENTS TO PUBLIC   |
| SERVICE ELECTRIC AND GAS COMPANY.                                            |

| THREE SPRINKLER SYSTEMS DECLARED INOPERABLE DUE TO CORRODED HEADS (24-hour   |
| report)                                                                      |
|                                                                              |
| The following text is a portion of a facsimile received from Paducah:        |
|                                                                              |
| "On 06/02/99 at 1630 CDT, the Plant Shift Superintendent (PSS) was notified  |
| that numerous sprinkler heads were corroded, affecting 16 sprinkler systems  |
| in C-337 and one system in C-333, such that the ability of the sprinklers to |
| flow sufficient water was called into question.  Subsequently, these         |
| sprinkler systems were declared inoperable, and TSR-required actions         |
| establishing roving fire patrols were initiated.  This deficiency was        |
| detected during scheduled system inspections conducted by Fire Protection    |
| personnel.  Currently, functionality of the sprinkler heads has not been     |
| fully evaluated by Fire Protection personnel, and the remaining cascade      |
| buildings are currently being inspected, and if necessary, this report will  |
| be updated to identify any additional areas.                                 |

| "Two sprinkler heads on system D-1 in C-337 and two sprinkler heads on       |
| system 27 in C-335 were identified to also be corroded.  These were          |
| identified to the PSS on 06/03/99 at 1600 CDT and 1601 CDT, respectively,    |
| and determined to require an update to this report by the PSS.               |

| in C-333 were identified to also be corroded.  The PSS was notified of this  |
| condition at 1300 CDT on 06/17/99 and determined that an update to this      |
| report was required."                                                        |

| "Two sprinkler heads on system C-15 and one sprinkler head on system B-8 in  |
| C-333 were identified to have corrosion.  The PSS was notified of this       |
| condition at 1350 CDT on 06/18/99.  The area of the fire patrol for system   |
| C-15 was expanded to include the two heads identified as corroded.  The one  |
| head on system B-8 was in the area already being patrolled.  The PSS         |
| determined that an update to this report was required."                      |

| "Two sprinkler heads on system D-8 and three sprinkler heads on system D-7   |
| in C-337 were identified to have corrosion.  The PSS was notified of the     |
| condition on system D-8 at 0125 CDT on 06/25/99 and at 1019 CDT on 06/25/99  |
| for system D-7.  Both systems were immediately declared inoperable and LCO   |
| fire patrol actions were implemented.  It was determined that an update to   |
| this report was required."                                                   |

| parts and declared the system inoperable at 0931 CDT on 07/30/99.  LCO       |
| required fire patrols of the affected area were initiated.  The PSS          |
| determined that an update to this event report was required."                |

| THE LICENSEE DETERMINED A CONDITION THAT WOULD HAVE PREVENTED THE CVC SYSTEM |
| FROM FULFILLING ITS INTENDED SAFETY FUNCTION.                                |
|                                                                              |
| At 1643 CDT on 8/2/99 with charging pumps 'A' and 'B' operating, the control |
| room received the "CHARGING HEADER FLOW LO" annunciator.  The operator       |
| observed that charging header flow indicated 0.0 gpm. The control room       |
| personnel selectively secured charging pump 'A' and observed that the        |
| charging header flow returned to the expected flow (44.0 gpm) for a single   |
| operating charging pump.  At 1650 charging pump 'A' was restarted to         |
| investigate if the condition was repeatable.  Operations personnel were      |
| observing the charging pumps locally during this period.  The condition was  |
| identified as repeatable, however, operations personnel could not            |
| conclusive identify locally any abnormalities.  Charging pump 'A' was        |
| declared inoperable and was secured in the "OFF" condition to preclude any   |
| unplanned starts due to automatic actuations (pressurizer level deviation or |
| SIAS) and Technical Specification 3.1.2.4 was entered at 1654 (CDT). The     |
| on-coming shift isolated and danger tagged charging pump 'A'.   After review |
| of the information by licensing personnel on August 3, 1999, it was          |
| determined that this failure may have prevented the CVC System from          |
| fulfilling its intended safety function.                                     |
| The plant was in a 72 hr LCO action statement to have both charging pumps    |
| operable while in modes 1,2,3, & 4.  The plant entered mode 5 within the 72  |
| hr action statement.  The cause of the problem is being investigated.        |

| The resident inspector will be notified.                                     |

| A team was appointed to investigate the event that was observed on August 2, |
| 1999. Plant  data and discussions with individuals were performed to         |
| identify pertinent facts and potential failure modes.  Additional            |
| troubleshooting was performed August 6, 1999 to confirm which components     |
| were operating correctly or possibly failed.  The result of the              |
| troubleshooting concluded that a failure of the charging flow transmitter    |
| was occurring at low pressures (indicator was sporadic).  The flow was       |
| verified by other plant parameters (i.e., temperatures, pressures, and       |
| levels) and by the use of a temporary installed ultrasonic flow meter.  The  |
| transmitter was removed from service and replaced.  The transmitter was      |
| tested by maintenance and the failure could be repeated.                     |
|                                                                              |
| The failure of the charging flow transmitter gave the indication that        |
| charging flow was lost but at no time during the plant evolution was actual  |
| flow lost.   Based on this information we are retracting the four hour       |
| notification under 10CFR50.72(b)(2)(iii)(D) that was made on August 3,       |
| 1999.                                                                        |

| BULLETIN 91-01 24 HOUR REPORT                                                |
|                                                                              |
| Operations personnel during a walkdown of the seal exhaust system discovered |
| that a nuclear criticality safety analysis (NCSA) requirement, maximum oil   |
| volume, could not be verified when an oil level was observed in the pumps    |
| separator's sight glass.  The pumps in question were used for the highly     |
| enriched uranium (HEU) program, and after it's suspension, prior to March    |
| 1997, the pumps were placed out of service.  Visible oil level in the sight  |
| glass without supporting documentation for oil capacity of these pumps       |
| constitutes a loss of one control (Volume) for double contingency.  The      |
| second control (Interaction) was maintained.                                 |
|                                                                              |
| There was no loss of hazardous/radioactive material or                       |
| radioactive/radiological contamination exposure as a result of this event.   |
|                                                                              |
| The licensee notified the NRC resident inspector and DOE.                    |

| COURTESY CALL REGARDING DISCOVERY OF FISH KILL IN CIRCULATING WATER INTAKE   |
|                                                                              |
| The licensee notified the Texas State Parks and Wildlife Agency regarding    |
| the discovery of several thousand dead shad in the vicinity of the           |
| circulating water intake. The state informed the licensee that this          |
| discovery was not reportable, hence the licensee is providing this           |
| notification to the NRC as a courtesy. The following information was         |
| provided by the licensee via facsimile:                                      |
|                                                                              |
| "Approximately two to three thousand shad were observed floating in the      |
| vicinity of the circulating water intake area. No sport fish were observed   |
| in this area. The likely cause of the die-off was stress to the shad due to  |
| dissolved oxygen depletion and high ambient temperatures. A survey of the    |
| main reservoir revealed few (less than ten) floating fish in the main water  |
| body. Consequently, this condition appears to be limited to the intake area  |
| only. A courtesy call to the Texas Parks and Wildlife was made at            |
| approximately 1100 on 08/07/99. The agency indicated that no formal          |
| reporting was required. A follow-up survey of the main reservoir on 08/08/99 |
| confirmed that only shad in the intake area remained affected and no         |
| floating fish were observed in the main water body of the reservoir. A       |
| walk-down of the intake structure on 08/09/99 revealed several thousand      |
| floating shad collected in front of the Unit 1 trash racks extending out     |
| approximately 10 feet from the trash racks."                                 |

| EMERGENCY DIESEL GENERATOR DECLARED INOPERABLE DUE TO FUEL OIL LEAK          |
|                                                                              |
| "Abnormal readings from the lube oil analysis of the division 2 diesel       |
| generator lead to further testing of its lube oil. Gas chromatography        |
| analysis of the division 2 diesel generator lube oil showed a 5.7% fuel oil  |
| dilution. Vendor recommendations were to not operate the diesel with greater |
| than 5.0% fuel oil dilution. The division 2 diesel generator was tagged out  |
| for maintenance and declared inoperable on 8/9/99 at 1330 central time.      |
| During maintenance of the division 2 diesel generator it was discovered that |
| the 12th cylinder of the 16 cylinder engine fuel injector return line        |
| fitting was weeping fuel oil.                                                |

| CONTROL ROOM PENETRATION OUTSIDE OF 10 CFR 50, APPENDIX R DESIGN BASIS       |
|                                                                              |
| The licensee has identified a 12" x 12" through-wall penetration between the |
| emergency switchgear room and the control room annex that was not filled in  |
| accordance with the licensee's fire barrier construction standards. The      |
| penetration was filled with mineral wool that did not meet the three hour    |
| fire rating requirements for the control room. The licensee has posted a     |
| continuous fire watch in this area as a compensatory measure. The NRC        |
| resident inspector will be informed by the licensee.                         |
