| -AFW PUMP STARTED DUE TO LOW S/G LEVEL SIGNALS WHILE VENTING A S/G LEVEL |

| WITH UNIT 3 IN MODE 5 (COLD SHUTDOWN) IN A REFUELING OUTAGE AND THE #2A |
| VITAL INSTRUMENT AC BUS INOPERABLE, PLANT TECHNICIANS WERE VENTING AN 'A' |
| STEAM GENERATOR (S/G) LEVEL TRANSMITTER. THE 'A' AUXILIARY FEEDWATER (AFW) |
| PUMP STARTED DUE TO A LOW S/G LEVEL ACTUATION SIGNAL ALONG WITH A LOW S/G |
| LEVEL SIGNAL DUE TO THE #2A VITAL INSTRUMENT AC BUS BEING INOPERABLE (2 OUT |
| OF 4 LOGIC ACTUATION SIGNAL). PLANT OPERATORS STOPPED THE 'A' AFW PUMP AND |
| RESET IT TO NORMAL WITHIN ONE MINUTE. |

| THE LICENSEE NOTIFIED STATE AND LOCAL OFFICIALS AND THE NRC RESIDENT |
| INSPECTOR. |

| During the event investigation it was determined that the ESF actuation of |
| the 'A' MDAFW Pump was not reportable because the signal was invalid and the |
| system had been properly removed from service and controlled in accordance |
| with the applicable operating procedures. Therefore, this condition did not |
| represent a reportable actuation of an ESF pursuant to 10 CFR |
| 50.72(b)(2)(ii) and 10 CFR 50.73(a)(2)(iv). The condition is also not |
| reportable pursuant to 10 CFR 50.73 (a)(2)(i)(B), as an event or condition |
| that is prohibited by the Technical Specifications (TS) because the Unit was |
| in Mode 5, a condition in which the TS are not applicable to this ESF |
| System. The Immediate Notification is therefore being retracted. The |
| licensee notified the NRC resident inspector. |

| AUTOMATIC START OF AUX BUILDING SPECIAL VENT SYSTEM |
| |
| The auxiliary building special vent system received an automatic start |
| signal from a spike on radiation monitor 1R30 (aux building vent gas |
| radiation monitor). Redundant radiation monitor 1R37 did not show any change |
| in radiation levels. The licensee will investigate the cause of the spike on |
| monitor 1R30. |
| |
| The NRC resident inspector has been informed of this event by the licensee. |

| |
| Further investigation found that the automatic actuation of the auxiliary |
| building special vent system was invalid. The actuation resulted from |
| confirmed spiking of the auxiliary building vent gas radiation monitor |
| (1R30). |

| The NRC resident inspector has been notified. |

| OFFSITE NOTIFICATION REGARDING ANOMALY DURING RADIOGRAPHIC PROCEDURE |
| |
| At approximately 0500 CDT, following the radiograph of a check valve, the |
| source could not be retracted into its shield using the normal method. |
| However, the radiographer was able to manually retract the source without |
| directly handling the source. The licensee notified the State of Mississippi |
| regarding this anomaly. The NRC resident inspector will be informed of this |
| notification by the licensee. |

| LOST TRITIUM EXIT SIGNS |
| |
| Anheuser Busch reported losing 4 exit signs at their facility located at 700 |
| Schrock Road, Columbus, OH. Three signs had 5.2 Ci and one had 7.9 Ci of |
| tritium, the age of the signs was not available. They were determined to be |
| missing on 7/6/99, and a subsequent search did not find the signs. |
| Anheuser Busch believes the signs were stolen. |

| LOST PACKAGE CONTAINING 4.9 mCi OF IODINE 131 |
| |
| A package containing 4.9 mCi of I-131 in the form of two NaI solid dosage |
| capsules was inadvertently picked up by the wrong courier from the |
| licensee's facility in New Brunswick, NJ. The courier was supposed to be |
| Delmed, but the package was in fact picked up by E. LBLINK. The licensee |
| continues to investigate this event, but does not yet know why E. LBLINK |
| picked up this package. The event was discovered when the courier from |
| Delmed arrived and requested the package. |
| |
| The shipment was intended for North Troy University Hospital in Manhasset, |
| NY. The hospital has been contacted and confirmed that the shipment has not |
| yet arrived. It was due at 1:00 p.m. today, 7/19/99. Discussions with the |
| personnel involved revealed the E. LBLINK courier understood the package was |
| to go to New York, even though the package was not intended for his company. |
| The license plate of the truck was recorded and is available in the NRC |
| Operations Center log. |

| The package was properly prepared and has an transportation index of 0.5. |

| EMERGENCY SIRENS INOPERABLE |

| A lightning strike has rendered the prompt alert system (emergency sirens) |
| inoperable. The licensee has notified the Massachusetts Emergency |
| Management Agency as well as the five towns in the Emergency Planning Zone. |
| The system should be restored by tomorrow morning. |
| |
| The NRC resident inspector has been notified. |
