| The licensee made the following 24-hour report. Unit 1 is presently |
| operating at 91% power. |
| |
| "A dead seal was discovered in Seabrook Station's Service Water Pumphouse |
| today. This 24-hour notification is made in accordance with Section 4.1 of |
| the Environmental Protection Plan, Appendix B, of the Operating License. The |
| National Marine Fisheries Service in Gloucester, Massachusetts, has been |
| notified under the Marine Mammal Protection Act. |

| - ONE CONTROL ROD INSERTED INTO THE REACTOR VESSEL WITH THE FUEL OFF LOADED |
| - |
| |
| While all fuel was removed from the Brunswick Unit 2 reactor vessel for a |
| scheduled refueling outage, an inadvertent reactor protection system |
| actuation resulted in the insertion of one control rod (#50-31). The |
| invalid scram signal was due to maintenance already underway on the "2A" DC |
| switchboard (removed from service under clearance, half scram signal present |
| on RPS "A" channel due to de-energization of reactor protection system |
| instrumentation) and an inadvertent high level trip of average power range |
| monitor B due to working on local power range monitoring instrumentation |
| underneath the reactor vessel (half scram signal on the RPS "B" channel). |
| The one control rod which moved had been withdrawn in preparation (or |
| placing it under clearance) for control rod drive replacement. Seven other |
| control rods were completely withdrawn and under clearance to support |
| inspection work inside the reactor vessel. These rods did not move during |
| this event due to being under clearance. This event has no safety |
| significance. There was no fuel inside the reactor vessel at the time of |
| this event. |

| The licensee is retracting this report based on the following: |
| |
| "This event does not meet this reporting criterion in that the actuation |
| signal resulting in the actuation of the RPS was invalid, and the safety |
| function of RPS had already been completed at the time of the occurrence. In |
| accordance with the technical specification, the RPS was not required to be |
| operable based on the condition of the plant at the time of the occurrence. |
| With the core defueled, the insertion of control rods no longer adds |
| negative reactivity, and therefore, the safety function of RPS had already |
| been completed prior to the occurrence. |
| |
| "The above discussion also applies to the 10 CFR 50.73 reporting |
| requirements. |
| |
| "Based on the above analysis, the red phone report (Event Number 35657) was |
| retracted by Steve Tabor on May 17, 1999, at 12:00 EDT. The NRC contact was |

| LOSS OF POWER SIGNAL ON THE 'A' TRAIN GENERATOR LOAD SEQUENCER |
| |
| A loss of power signal occurred when the licensee restored the 'A' train |
| generator load sequencer as part of restoration from an 'A' train bus |
| outage. At the time, most of the major components were tagged out of |
| service and deenergized because the unit was 'B' train protected due to the |
| 'A' train outage. However, some of the smaller components (valves and |
| dampers) did move when the loss of power signal occurred. The licensee |
| believes that the signal was valid because the sequencer remembered that the |
| bus was deenergized earlier this evening. |
| |
| Nothing that actuated affected the licensee's ability to cool the core or |
| the spent fuel pool. In addition, anything needed by technical |
| specifications was not affected. Therefore, the licensee believes that the |
| affect on the plant was minimal. All systems functioned as required, and |
| all affected components have subsequently been restored to normal. |
| |
| The cause of this event is currently under investigation. At this time, the |
| licensee hypothesizes that either there was something tagged out that was |
| confusing the sequencer or that the procedure sequence did not reset the |
| sequencer memory. The licensee stated that the procedures were followed |
| step by step. It was also noted that the procedure had a large revision bar |
| and that something may have been left out. The licensee plans to |
| investigate whether or not the procedure step sequence was proper. The |
| licensee also plans to correct this problem before the unit enters Mode 4 |
| (when the sequencer is required to be operable by technical specifications). |
| The current estimated restart date is 06/15/99. |

| The licensee notified the NRC resident inspector, the applicable state and |
| local agencies, and the Department of Environmental Protection. |

| LOST OR STOLEN 3-CURIE TRITIUM SOURCE FROM AN M140 ALIGNMENT DEVICE |
| |
| Sometime on 5/16/99 near Watervliet Arsenal (about 35 miles from Albany, NY) |
| at a contractor's facility (Wright Malta Corporation at Wright Malta |
| Station), a break-in occurred. In the facility was a howitzer with an M1A1 |
| collimator (10 curies tritium) and an M140 alignment device (3 curies |
| tritium). The two devices were taken but were recovered on site when the |
| thieves were interrupted on the way out and dropped them. However, the |
| source was missing from the M140 alignment device, and it is unknown whether |
| the source was missing previously or removed and taken by the thieves. The |
| local police are following up on the break-in and theft. The Watervliet |
| Arsenal contacted the State of New York, who in turn notified NRC Region 1. |

| MANUAL REACTOR TRIP FROM 73% POWER DUE TO UNEXPECTED HIGH MAIN TURBINE |

| While the licensee was in the process of increasing load after exiting a |
| refueling outage, vibrations on the #1 and #2 bearings for the main turbine |
| were noted to be increasing unexpectedly. As a result, operators began to |
| decrease load at 0036 on 05/18/99 with the unit at approximately 83% reactor |
| power. Operators manually tripped the reactor from 73% power at 0041. At |
| that time of the manual reactor trip, vibrations on the #1 and #2 bearings |
| were at a level of about 10� mills and 12 mils, respectively. The limits on |
| vibration are either 10 mils for 15 minutes or when a vibration of 12 mils |
| is reached. |
| |
| All control rods fully inserted as a result of the manual reactor trip, and |
| all systems functioned as required. There were no engineered safety feature |
| actuations or emergency core cooling system injections, and none were |
| required. |
| |
| The unit is currently stable in Mode 3 (Hot Standby). The reactor coolant |
| pumps, normal charging and letdown, and pressurizer heaters and sprays are |
| being utilized for primary system transport, level, and pressure control. |
| All containment parameters are normal. Emergency feedwater was manually |
| initiated and is currently supplying water to the steam generators. |
| Secondary steam is being dumped to the condenser. Offsite power is |
| available, and the emergency diesel generators and emergency core cooling |
| systems are operable and available if needed. |
| |
| The licensee notified the NRC resident inspector. |
